I am 51 y/o female, 6 months post full facelift. What should my regimen be to maintain the
Maintaining a facelift
May 29, 2019
Great question! A facelift can "set the clock back" but it does not stop the aging process. There are few things that happen with aging that can affect your results. Your skin will loose elastin and collagen which can cause wrinkles or sagging. Using sunscreen and avoiding the sun can help protect the collagen and elastin in your skin and protect your investment. Using a daily facial moisturizer, drinking plenty of water and healthy diets can...
The tip of my nose is very swollen, what else can I do that is non-surgical to help the sw
Swelling of tip of nose after rhinoplasty in a diabetic
May 28, 2019
It is difficult to tell if there is still swelling without comparing it to the pre-operative photos. It looks as if you have thick skin to the nasal tip which can limit the amount of tip refinement that you can achieve with rhinoplasty. I would continue to follow up with your surgeon for his/her recommendations. As far as the steroids go in a Type II diabetic, I would not be worried about it having a significant affect on your blood sugar. It is used in...
SMAS face and neck lift 2 years ago, platysma bands showing, advice?
Platysmal bands after a facelift
May 17, 2019
A facelift does not freeze the face in time. I do not know if those were present originally or if they developed afterwards and would have developed over time naturally. The good news is that they can be treated through a small incision just under the chin. This can be done in the office or the operating room, depending on surgeon and patient comfort levels. Botox or Radiofrequency treatments will probably not get you the results you are looking for.
What is wrong with my chin area after SMAS and fat grafting?
Lack of result after SMAS lift
May 17, 2019
From the photos, it looks like the tissues did not hold as expected. Performing a revision SMAS facelift could potentially get you the results you are looking for. I don't believe more fat will get you the results of a more smooth and continuous jawline. I hope this helps.
Could these stitches left in my ear post facelift be causing my pain and swelling? (Photos
pain from suture
May 17, 2019
If the pain is located just to that spot, there may be some chronic irritation that could cause some pain. It is not uncommon to have pain just under the earlobe. There is a nerve that runs up toward the ear there and with the sutures from the facelift it can pinch the nerve and would be unrelated to the sutures shown in the photo. Your surgeon should be able to remove the possible piece of suture shown in the second photo. Otherwise a small dose of...
1 month post-op. Will my nose get smaller? Will the tip drop?
One month post rhinoplasty
Apr 17, 2019
It is difficult to discern on the photos, but your tip looks more narrow and will likely continue to improve. The hump has been removed and no obvious hump remains from what can be seen in the photos. You may feel that the dorsum is wide because a hump makes the nasal bones appear more narrow from the frontal view. But, it does look like it was narrowed from the photo. As far as the tip dropping with time, if a...
1 month post-op. Is taping beneficial now since my nose is still sensitive and throbbing?
Taping after a rhinoplasty
Apr 17, 2019
Thank you for your question. Taping can help to minimize the swelling that occurs post rhinoplasty. How long you tape is surgeon dependent. Taping for 6 mos is not common but if Alloderm was used I have seen swelling last longer than normal. This may be why the 6 mos recommendation was given. If you have irritation to the skin from the tape, you may want to discuss the risks and benefits with your surgeon. In general, taping should not...
12 days post-op, is it safe to clean my nose with a q-tip?
Cleaning nose with a Q-tip
Apr 17, 2019
As long as you are not going deep into the nose or pushing on anything with the Q-tip, it should be fine. But, it's always best to check with your surgeon. Typically, saline rinses are needed after a rhinoplasty to rinse out any blood clots. The diluted peroxide is good for cleaning dried blood off the incisions (with a Q-tip) but not great for irrigating the nose. Hope this helps
Does my septum look deviated?
crooked septum
Jan 25, 2019
If you are not having nasal breathing problems then there are no worries unless you are concerned about it from a cosmetic standpoint. Your septum is actually inside your nose. I believe the area you are concerned about, giving the appearance of asymmetric nostrils is the footplate of your lower lateral cartilage. These sit right next to septum. You may have a small crooked part of the septum pushing the footplate into the nostril or it may just be the way the...

Balance after rhinoplasty
Jan 22, 2019
From the picture it looks like you have a deficiency of your radix, giving you the illusion of a larger hump. Your nose only appears slightly over projected. With a long face, if you only take down the hump and deproject the tip, you may end up with a nose that does not match your face. If you are concerned about the surgical aspect, filler could be used for a non-surgical rhinoplasty to fill in the radix. I hope this helps!
Is it common to have a rhinoplasty and otoplasty at the same time?
Rhinoplasty and Otoplasty
Jan 22, 2019
The short answer is yes, they can both be done at the same time. They are both common surgeries but are less common to be performed together. A chin implant is much more common to be performed at the time of a rhinoplasty. The advantages of having them done at the same time is the one recovery, and the OR costs. As far as results you are looking for, these are best discussed with the surgeon and he/she can work with you to get the results you...
Can I get cheekbone filler and have a dermal anchor piercing?
Filler and dermal anchor piercing
Jan 22, 2019
I don't do anchor piercing, however, I hope I can help you with your question. Cheek filler is typically placed deep, just over the bone. This should be away from the piercing. I would recommend having the filler placed at least a couple weeks before the dermal anchor. You need to be careful because cheek filler is a foreign body (even though most are hyaluronic acid which is found in the body) and if the piercing is not done under sterile technique,...
Is a 2 hour flight 2 days after chin liposuction a DVT or PE risk?
DVT risk during flight after neck lipo
Jan 21, 2019
Neck liposuction under local anesthesia is a safe surgical procedure. All surgical procedures, in general, can increase risk of a DVT but this one would have very little increase in risk. If you have a history of a clotting disorder, this would be more of a concern even if you did not have surgery. Always clear your plans with your surgeon as he/she may know something in your history that would restrict you from flying that soon afterwards. Otherwise,...
How sun sensitive does the nose become after alar base reduction long term?
Sun after alar base reduction
Jan 9, 2019
An alar base reduction should not make your nose sensitive to the sun. Noses, overall, can be sensitive to the sun and our sun sensitivity can change throughout life. Scars can tan differently than surrounding tissue and may be more or less prominent with a tan. It is a good idea to use sunscreen on your nose and areas of scars even years after surgery. I hope this helps!

Multiple procedures with general or without general anesthesia
Jan 8, 2019
The short answer is yes it can be done under general anesthesia, local with IV sedation or local with oral sedation. All are safe under trained hands. General anesthesia has the advantage of allowing the surgeon to move more quickly as you will not be moving. The multiple procedures just add to the anesthesia time. Your surgeon may offer the procedure in office or the OR with IV sedation and local (injected anesthesia). Or, he/she...
