You should be an excellent patient for an endonasal(closed) preservation rhinoplasty. your skin appears to be relatively thin with decent lower lateral tip cartilage. you would like need suture and cartilage recontouring of these tissues. the recovery would be rapid with modest swelling. the definition of the tip in a male rhinoplasty is key to a good looking result and your should be able to get a nice result

There are two areas that would make your nose look more attractive. a small bump on the bridge could be removed.this must be done very conservatively so that it doesn’t give the illusion that the tip is larger. the tip should be reduced significantly in size and increased in definition. this would be done endonasally using micro sutures as well as contouring cartilage grafts. alar skin wedges would reduce the size of the nostrils.

Looking at your photo there are a couple of areas of the nose the could use some further refinement . the dorsum is somewhat wide and could be slimed dome a small amount. the tip shows that the columella is slightly droopy and there is a lack of definition within the apex of the tip. as they say beauty is in the eyes of the beholder so its really your call if you would want to undergo further surgery to refinement your nose.

Interesting question….surgical shortening the upper lip can be challenging. unfortunately surgical shortening this area will always result  in a very visible scar beneath the nostrils. i therefore do not recommend this procedure .you may improve this area by recontouring the columellar area beneath the nasal tip to get a better balance between the nasal tip and the lip, especially when you smile

It appears that you had 2 tiplastys that resulted in a uneven result. the cause was most likely improper contouring of your lower later cartilages in the first place. this resulted in a collapse of the facet area. filler will never permanently correct this, the cartilage surrounding the area of depression must be corrected and properly reconstructed.sometimes a small crushed cartilage graft placed within the facet depression would correct and residual deformity.

At 8 days after the surgery the shape of the nose should be fairly stable. The nose tissue is not like clay, which could be easily deformed by minor pressure on it. The cast that was on your nose provides protection while it is on. After it is removed the nose has already undergone a significant healing process. There is no question that there is a modest amount of pre-tip edema (swelling). I personally would wait 10-12 months before assuming this will not resolve over time. However, a common underlying cause of the rounded appearance of the poly-beak is typically because the tip of the nose lacks adequate projection after it has been reduced in size during a primary tiplasty. Two weeks after the surgery your nose should be reasonable well healed. A very modest intake of alcohol can normally be taken at this point. We are talking 1 – 2 beers and 1 shot of vodka type liqueur should not affect the healing at this point.

Absolutely as long as there is not too much swelling from the filler treatment. The profile contour of the nose can be judged because swelling from the filler should not affect the assessment of how much of bump should be removed during the rhinoplasty.
