Fixing a deviated septum is a pretty routine part of my rhinoplasty procedure, especially when it helps me to get the cartilage grafts that I need to finish the job. If the aesthetics of your nose bother you, it is best to get the rhinoplasty done at the same time as the septoplasty so that that cartilage material doesn’t go to waste.

The two main things that I evaluate on profile are the chin projection and the nose. Your chin has adequate projection, but your nose would benefit from refinement by lowering the bridge and raising the tip slightly, while keeping it very natural appearing. This can all be done with closed rhinoplasty.

I would not focus on closed vs open rhinoplasty. You can be helped with closed rhinoplasty as well to create more of a natural flow between the different areas of the nose and address your concerns. Speak further with a nose expert to determine your surgical goals and begin to understand what can and cannot be accomplished with this surgery the second time around.

Sorry to hear about your breathing issue after rhinoplasty. This can occur due to a number of reasons that range from persistent swelling and scar tissue to graft displacement and untreated deviated septum. You should continue to see your surgeon but, if you are not getting the relief or the answers that you need after a few tries, you might want to consider a second opinion. You would need a good in person physical exam to figure out what is going on in this case. Best of luck!

For the long term, closed rhinoplasty would be the best option to take down the bump. Filler will allow you to disguise the hump by raising the root of the nose, but won’t make the bump go away.
