Rhinoplasty without osteotomy
It is possible to remove the hump without an infracture, though as the bridge is lowered the nose can tend to widen, and the infracture can control the width.
If gently performed you will be little worse for wear and it does give your surgeon the flexibility to keep things in balance. (Peter E. Johnson, MD, Chicago Plastic Surgeon)
Is it possible to shave the hump in my nose without an Osteotomy?
Yes, a subtle reduction of your bridge is possible and likely would not require osteotomies. However, this would not create the best possible result.
There would definitely be a trade-off to avoid osteotomies which are feared by some but typically do not create any significant increase in downtime or surgical risk. (Stephen Weber, MD, FACS, Denver Facial Plastic Surgeon)
No osteotomy rhinoplasty.
This is possible if you have limited expectations of how much reduction of the hump you want. The osteotomy’s are only used to bring the bones together but if the bones are not separated this is not needed. (Toby Mayer, MD, Beverly Hills Facial Plastic Surgeon)
Is it possible to shave the hump in my nose without an Osteotomy?
The answer this question, is yes, you can have the bump removed without doing osteotomy. The amount of the reduction will be limited but it can be reduced. (Francis R. Palmer, III, MD, Beverly Hills Facial Plastic Surgeon)
Hump removal
What you really need to know is what will give you the best result and that can only be determined with a full examination aided by digital photos (Frederic H. Corbin, MD, Brea Plastic Surgeon)
Rhinoplasty without osteotomies
Dorsal shaving may improve the shape of your nasal dorsum somewhat but the amount of reduction would be limited. The more you take from the dorsum the more likely you are to need osteotomies to reshape the bony structure of your nose.Think about it like a triangle.
If you take off too much of the tip of the triangle you have to close the top again by angling the walls in (requiring an osteotomy). If you minimize how much you take off you may not need it. Best to schedule a consultation and discuss your concerns in detail. (Steven H. Williams, MD, San Francisco Plastic Surgeon)
Patient’s Shouldn’t Tell the Surgeon How to Operate
You should have a thorough discussion of your issues with an experienced surgeon. Although it is unclear why you don’t want osteotomies, the fact of the matter is that they are necessary to achieve a normal appearance after any significant hump reduction. Osteotomies are just one a many maneuvers that are performed during a rhinoplasty, however it has become a very well known part of a rhinoplasty amongst lay people.
There are many other surgical maneuvers that might seem just as objectionable to some people, but because they are unknown, people do not request their omission from the surgery. The bottom line is the technical aspects of surgical procedures can be psychologically unpleasant, and that is unavoidable. A prospective patient should focus on more relevant issues like risks and benefits of a surgery, and less on the nuts and bolts of how the surgery is performed. (Gerald Minniti, MD, FACS, Beverly Hills Plastic Surgeon)

You would need fractures of the nasal bones to avoid an open roof deformity. (Darryl J. Blinski, MD, Miami Plastic Surgeon)
Shave Nasal Hump without Osteotomy
The amount of hump removal will be limited if you do not want osteotomies. It can be reduced but I would not predict how much on the basis of a picture. This should be discussed at the time of consultation. (Richard W. Fleming, MD, Beverly Hills Facial Plastic Surgeon)
Is it possible to shave the hump in my nose without an Osteotomy?
There are several ways to approach a dorsal hump in your case. Radix augmentation in combination with shave may be possible, but a frontal view would be helpful to make a determination about osteotomy as well. (Kenneth B. Hughes, MD, Los Angeles Plastic Surgeon)
Decreasing a hump without using osteotomies
It is hard to know whether just taking the hump down is possible without creating what we call an open roof deformity. Granted, the change you simulated is relatively modest, but it can be the “straight-on” view where a lack of osteotomies to “close the open roof” can be most apparent. Think of a pyramid (your hump) where you take down the top. Now you are left with a trapezoid or a flat spot on the top. The more you take, the more important it is to do osteotomies to recreate that pyramidal / triangular configuration. That being said, go to an experienced rhinoplasty surgeon who will decide on the best technique for your surgery to get you the best result. (Michael M. Kim, MD, Portland Facial Plastic Surgeon)
Hump & osteotomy
It is difficult to say without seeing a front view of your nose. The tradeoff of lowering without osteotomies is that the more you lower it, the wider and flatter the bridge of the nose becomes. The extreme of this is the “open-roof” deformity. Not everyone requires osteotomies, but most people do in order to retain pleasing proportions. (Robert H. Hunsaker, MD, Miami Plastic Surgeon)
