I see so many young people for rhinoplasty and this is a very common question that families have. To be honest, our best information in the science literature would suggest that the female facial skeleton and structures are mature at age 15. In males this is usually considered to be around 17.

Usually I will consider a number of items including recent growth history, family history, overall skeletal maturation, and areas of the body known to mirror the facial development in parallel such as shoe size. More importantly, we have to consider the maturity and understanding of the patients emotional development. Rhinoplasty should be remarkably reliable, but we do see revision rates in the United States of up to 15%. Another important consideration is healing time and lifestyle.

It seems to work very well for my patients to undergo a rhinoplasty just after graduation from high school before they move on to the next phase in life. A very good discussion and rapport is required prior to undertaking such an important decision. I usually will recommend at least 2 other doctors in my area as second and third opinions for all of these patients as well before we actually operate. Age is not the limiting factor for performing a rhinoplasty procedure safely. For both women and men, the growth and development of the midface is dependent on the septum (or midline) of the nose. Any work on this area will affect the growth of the face. Research has shown that nasal bone work (osteotomies) or nasal tip work is safe and should not affect the development of the nose and face if done before puberty. Septum work (correcting a deviated septum) or obtaining septal cartilage for grafting during rhinoplasty, however, will affect future face development and thus should not be done before puberty or the age of 17 in men. Most women, though, have had their first menstrual cycle and growth spurt before the age of 17 while men can continue to grow until the age of 18-21.
