From your photos, it would appear that you have what is referred to as moderate to sever ptosis of your breasts.( saggy boobs). This requires a breast lift. Unfortunately, you have such a large volume to fill in your breasts it is unlikely, any of the current sized implants alone would be able to provide you with upper pole fullness. I am not sure what that is. I saw something similar for the first time a couple of weeks ago on one of my patients. I actually excised it. I thought I might see a piece of suture. On gross exam there was no evidence of suture material. It might be a local response to a previous placed suture. Make your doctor aware of it. Maybe he or she would suggest a biopsy as well. It is not uncommon to see ladies with this issue.. Everyone is different, what works for my patients may not work for you. However, I tell my patients I call this, “the mad tinglies”. It is a neurlogical response. I have found that if the patients massage the area where it occurs with lots of moisturizer, it seems to help. Just rubbing it when it occurs seems to help as well. This is a tough question. I think you need to do some reading regarding the implant position with respect to the formation of capsule contracture. My exposure the literature and the 5,250 breast augmentations I have done is that the incidence of capsule contracture in the same above and below the muscle. There is at least one very entertaining you tube video of a lady flexing her sub-muscular implants to the music. Most folks I work with don’t have that as a goal….

You have left off some very important information regarding your implants, where are they located; above or below the pectoralis muscle. Before having a breast lift I would consult with a ASPS Board Certified plastic surgeon. Every certified plastic surgeon has their own technique. In my experience of over 5,250 breast augmentations, I have seen this occur with the placement of the implant in the sub-pectoral position. 5 weeks after augmentation and my left nipple is too high. Should I worry?

Yours is a very common issue, breast asymmetry. You had the same relative nipple position before your surgery. Assuming those are your pre-op photos as well. Take a ruler and draw a line straight down from the indention in your neck ( the sternomanubrial notch) to your belly button. Then measure the distance from the midline you drew to each of the nipples. You will see that the left is longer than the right. This difference is exaggerated when you put an implant behind the breast…
