This is very hard to answer with an exam or photos. However, in my experience I’ve been able to close a full tummy regardless of torso length. I’m not a fan of a float because the belly button looks abnormal afterwards and is too low. If yours is too high, then maybe that would be okay. Make sure you review this very carefully with your surgeon and consider a second opinion.

I would agree with the other physicians. The silicone implants are going to feel much more natural than a saline implant. With the minimal amount of breast tissue you have, that will be more important. In general the implant material becomes more important, and needs to be more natural and feel, as your implant to breast ratio exceeds 50/50. That is just a rule of thumb. But I generally recommend silicone because they are overall a superior implant.

A large hernia such as the one you are showing should be repaired before a tummy tuck. However, I would consult with a plastic surgeon and general surgeon before undergoing either surgery. Be sure that they collaborate before your surgery. What you don't want is the general surgeon to create a scar that cannot be removed with the tummy tuck. Some plastic surgeons also repair very large hernias, but not all. Make sure everyone is in communication so that your final goal is met.
