If the nose is to be de-projected, typically the entire nose will be reduced. If significant de-projection is required and the dorsum is not reduced, it may create a relative disharmony of the profile, with the de-projected tip accentuating a non-operated dorsum. This would place the patient at high risk for dissatisfaction with the result if correction of one problem creates a relative increase in the hump on profile.

Your concern is very common in inverted T reductions. The intersection of the incisions tends to be not only a point of higher tension, but also a distal point of the skin with the least vascularity. These issues can combine to create a delay in healing, but it usually heals within a few weeks with simple dressings like dry gauze or a band-aid.

A cephalic trim is a technique used to refine the tip of the nose. Cephalic refers to the upper portion of the lower lateral cartilages, and by resecting that upper portion, the tip of the nose can be lifted and narrowed. One must by careful to leave some of the lower lateral cartilages intact, because too much resection can lead to airway problems and a pinched tip.
