There are three important elements that need to be addressed here – Shape, size and herniated nipple areolar complex.As for shape, release of constriction, particularly at the bases will help control shape and symmetry. As for size, while fat graft is a viable option, it is somewhat unpredictable and based on your height and weight, and appropriate size implant may afford both improved shape and symmetry. Thirdly, a periareolar mastopexy will address the puffiness. Thank you.

1. Agree that there is a need for graft to correct the deviated nose2. Disagree that the nose has to look wider. This may need more selective osteotomies to prevent widening of the dorsum3. Breast augmentation may exaggerate any natural asymmetries or nipple malpoaition one may have. Chosing a smaller implant may be the key in avoiding over-exaggeration of these issues. Without adequate soft tissue coverage, implant palpability, capsular contracture, rippling etc happen more frequently. Please consult board certified plastic surgeons for appropriate guidance. Best.

A combination of liposuction (of any kind) combined with an RF-based skin tightening treatment (i.e. Thermi-Tight) would yield the best result. As you may be aware, Coolsculpting will help resolve “fat issue” but may exacerbate problems related to excess skin. Hence, following Coolsculpting, you may require a skin tightening procedure. Also be aware that results after Coolsculpting may not be obvious for several weeks.

In plastic surgery, particularly in breast surgery, there’s always a compromise – shape vs scar. In other words, if you want a better and longer lasting shape, you have to be willing to accept more scars.  Yes, a Benelli Mastopexy can be done with a permanent suture to keep the areola from expanding but long term stability in terms  of shape, may still be an issue. As you may be aware, choosing the right implant for a natural look depends on a whole host of factors. Most important ones are however,  breat measurements and quality and quantity of breast tissue present. And I agree that it should be consistent with your frame.in my experience, on a smaller frame, a smaller HP implant may give a more natural enhancement. Best.

Hi there, 4.5 weeks is too soon to start considering  any  revisions unless the asymmetry is due to a large size hematoma. It is not unusual to have asymmetric swelling that may resolve on its own. Yes, most of your concerns can be addressed surgically.  My recommendations would include reducing the hump, narrowing the bony base, repostioning and narrowing the tip and alar rim grafts to lower the nostrils.
