You would benefit from a cleft lip and nose revision.  Cleft revisions require an understanding of how to create symmetry.  Both your nose and the upper lip are asymmetric.  It also appears that the left side of your nose is pinched too much.  I would advise you to see another pediatric plastic surgeon, who has a lot of experience in treating cleft lip and nose deformities.  Good luck!

First, getting tear trough filler is usually a bad idea and often creates an artificial fullness.  This can be reversed with an enzyme called hyaluronidase. Second, you would benefit from a blepharoplasty but it has to be done right. If you make the crease too high you will probably not like it. I would advise you to see a board certified plastic surgeon to give more detailed information. Good luck!

Your implant appears to be too large and may not be the right shape.  You would most likely benefit from removal of the implant for an exchange to a different nasal implant.  There are several different implant shapes and sizes.  Also, there are implants that are L shaped that are designed to improve nasal tip projection.   However, these L shaped implants can make the nasal tip even larger than what you had before any implant surgery.  It is difficult to tell from just a side view.  It wo…

Even if a revision rhinoplasty is trying to improve upon a prior one, it is important to know what was done, what approach was used, and how much time has passed. A revision rhinoplasty will help if done properly.  You have several problems that you’ve correctly identified and could be much improved.  It would be best to be evaluated in person by an experience rhinoplasty and revision rhinoplasty surgeon to go over all of what could be done.  I normally perform open rhinoplasty for primary and revision rhinoplasty, because this approach allows me to be very precise and make all the appropriate sculpting techniques under direct visualization.  Good luck!

It is rare to add anything to the radix of the nose (the part of the nose above the dorsal hump). Looking at your before picture, it seems that proper correction of the dorsal hump without adding anything would’ve been my recommendation. It seems that you are getting different recommendations. An inside Weir excision makes no sense to me. Your main problem is that your nasal tip is very large and bulbous. It’s hard to believe that you needed ear cartilage graft in the first place. Whatever was done needs to be corrected and this would require a revision rhinoplasty by an experienced rhinoplasty surgeon. You appear to have excess lower eyelid fat that is herniating out. This would need to be confirmed with an examination in person. If you have excess fat, then you would benefit from lower eyelid blepharoplasty to remove the lower eyelid fat. You have volume deficiency to the anterior cheeks. A subtle improvement would help. I would recommend Sculptra and NOT Juvederm or Restylane for this. Be careful about tear trough injections. It certainly looks like you sustained facial fractures to the left infraorbital rim and probably the orbital floor known as a blowout fracture. You may have additional fractures to the maxilla. You will need a CT scan of the facial skeleton with fine cuts to determine exactly what is damaged. These fractures can be fixed but make sure you to a board certified plastic surgeon with a lot of experience in treating facial trauma. Good luck.

Rhinoplasty and revision rhinoplasty are not easy to do well. You have a residual mild dorsal hump. Your tip is asymmetric. So it’s more than rotation of the tip and decreasing projection. If you’ve found a fantastic rhinoplasty surgeon, then the doctor should be able to explain what exactly needs to be done and the precise technical methods that would be used to correct the problems that you have.
