Can anything be done to make my cleft lip more symmetrical and to even out my nostrils?
Revision will be beneficial but only if performed by an experienced cleft surgeon
Jan 7, 2019
You would benefit from a cleft lip and nose revision.  Cleft revisions require an understanding of how to create symmetry.  Both your nose and the upper lip are asymmetric.  It also appears that the left side of your nose is pinched too much.  I would advise you to see another pediatric plastic surgeon, who has a lot of experience in treating cleft lip and nose deformities.  Good luck!
Would you recommend me an upper blepharoplasty?
Bleparoplasty if done properly would help
Jan 4, 2019
First, getting tear trough filler is usually a bad idea and often creates an artificial fullness.  This can be reversed with an enzyme called hyaluronidase. Second, you would benefit from a blepharoplasty but it has to be done right. If you make the crease too high you will probably not like it. I would advise you to see a board certified plastic surgeon to give more detailed information. Good luck!
Nose bridge doesn’t look right – Any advise or tips?
Implant is not the right size!
Jan 3, 2019
Your implant appears to be too large and may not be the right shape.  You would most likely benefit from removal of the implant for an exchange to a different nasal implant.  There are several different implant shapes and sizes.  Also, there are implants that are L shaped that are designed to improve nasal tip projection.   However, these L shaped implants can make the nasal tip even larger than what you had before any implant surgery.  It is difficult to tell from just a side view.  It wo…
Do I need to tell my surgeon about my first nose job?
Yes, and it matters.
Nov 13, 2018
Even if a revision rhinoplasty is trying to improve upon a prior one, it is important to know what was done, what approach was used, and how much time has passed. An experienced rhinoplasty surgeon will want this information to give you the best guidance and technical judgment to improve the nose the way you want. It’s more important to get the revision done properly. Obviously, cost is a concern but I think there are enough plastic surgeons like me who don’t up charge for a revi…
What results can I reasonably expect?
Revision rhinoplasty with an open approach would be best.
Oct 31, 2018
A revision rhinoplasty will help if done properly.  You have several problems that you’ve correctly identified and could be much improved.  It would be best to be evaluated in person by an experience rhinoplasty and revision rhinoplasty surgeon to go over all of what could be done.  I normally perform open rhinoplasty for primary and revision rhinoplasty, because this approach allows me to be very precise and make all the appropriate sculpting techniques under direct visualization.  Good luck!
Can I do a supratip break to make the nose more feminine and to shorten it?
Need to remove dorsal hump!
Aug 21, 2018
It is rare to add anything to the radix of the nose (the part of the nose above the dorsal hump). Looking at your before picture, it seems that proper correction of the dorsal hump without adding anything would’ve been my recommendation. In the after picture, there is incomplete correction of the dorsal hump, which makes it seem like your nose is out of balance. Your nose is not long. Your nasolabial angle is nice. I agree with you that a properly done supratip will make it even cuter. N…
I have ear cartilage in my nose very high up. Do you think the weirs incision inside NOT outside would e
Be careful.
Aug 13, 2018
It seems that you are getting different recommendations. An inside Weir excision makes no sense to me. Your main problem is that your nasal tip is very large and bulbous. It’s hard to believe that you needed ear cartilage graft in the first place. Whatever was done needs to be corrected and this would require a revision rhinoplasty by an experienced rhinoplasty surgeon. I would recommend getting another opinion by a board certified plastic surgeon with experience in revision rhinoplasty to i…

Blepharoplasty to remove lower eyelid fat.
Aug 3, 2018
You appear to have excess lower eyelid fat that is herniating out. This would need to be confirmed with an examination in person. If you have excess fat, then you would benefit from lower eyelid blepharoplasty to remove the lower eyelid fat. You have volume deficiency to the anterior cheeks. A subtle improvement would help. I would recommend Sculptra and NOT Juvederm or Restylane for this. Be careful about tear trough injections. That would be a bad idea!  Good luck!

Facial fractures.
May 15, 2018
It certainly looks like you sustained facial fractures to the left infraorbital rim and probably the orbital floor known as a blowout fracture. You may have additional fractures to the maxilla. You will need a CT scan of the facial skeleton with fine cuts to determine exactly what is damaged. These fractures can be fixed but make sure you to a board certified plastic surgeon with a lot of experience in treating facial trauma. Good luck.
Revision rhinoplasty to reduce the size of the tip of my nose: I’ve been told this sort of surgery is re
Revision rhinoplasty is not easy.
May 15, 2018
Rhinoplasty and revision rhinoplasty are not easy to do well. You have a residual mild dorsal hump. Your tip is asymmetric. So it’s more than rotation of the tip and decreasing projection. If you’ve found a fantastic rhinoplasty surgeon, then the doctor should be able to explain what exactly needs to be done and the precise technical methods that would be used to correct the problems that you have. Good luck!
I’m totally confused. I want to get Sculptra to add volume to my cheeks and jawline
Sculptra, a wonderful product! However, if the doctor can’t explain how it works then find someone who can.
Apr 24, 2018
Sculptra is a great product if it is used properly.  That is true for any filler.  There is no formula for how much Sculptra you would need.  It would be based on what would be the best outcome for you.  When I see your pictures, you would most likely need two treatments (one vial per treatment, each treatment 4-6 weeks apart).  Sculptra comes as a powder and water is added to break up the powder into an even suspension.  The amount of water added to the vial of Sculptra is important.  The w…

A revision will help.
Apr 24, 2018
It is possible to improve upon the eyelid folding you have now to something better.  The shape of your left upper eyelid is not a good one.  It appears too high centrally and there is a scar that seems to be creating an extra folding pattern.  However, there is an important distinction between improving the eyelid folding vs. having someone else’s natural fold pattern.  Also, it won’t work without creating a full upper eyelid incision because the eyelid fold needs to be lowered with a mo…
Rejected for a neck lift! Surgeon said my mylohyoid are sagging, advice?
Face and Neck Lift will help!
Apr 20, 2018
Based on your photos, you seem to have the typical problems in the lower face and neck that would benefit from a face and neck lift.  I think you should seek another opinion by a board certified plastic surgeon, who will be able to examine you directly and give you an honest opinion.  Good luck!
What would be best in my case, a non surgical or surgical nose correction?
Revision rhinoplasty would be best.
Mar 27, 2018
Fillers would be a bad idea for the long term!!  First, filling the depressed side of your nose on the left will not change the right side or the nasal tip that could benefit from more correction.  It will be more costly over a long time period, since it will require annual or biannual injections.  Also, it could lead to other problems, such as occluding the arterial network that supplies blood to the skin of your nose.  Revision rhinoplasty would be the best option performed by an expe…
I saw a doctor about an upper eyelid lift but he suggested that I should also get a brow lift.
What do you want to achieve with a brow lift and upper eyelid blepharoplasty?
Mar 27, 2018
It is important to know what you want to achieve with an eyelid procedure or brow lift.  Why?  Because there are other aspects of your aging face that, if corrected properly, may have a much more important impact than your eyelids and brow hairline position.  Your brow hairline is asymmetric along the medial aspect and this could be from poor Botox injection.  Your lateral brow position is good and does not need any lifting.  Normally, we are trying to raise the lateral third and you don’…
