Liposuction is commonly performed with a tummy tuck. I have been using the lipoabdominoplasty technique exclusively for the past 15 years. This preserves the major blood supply to the abdominal wall, which improves the circulation, making complications less likely. It results in better contour and avoids the need for liposuction at another time. If a traditional tummy tuck is performed without preservation of the blood vessels from the muscle, liposuction could increase complications. 
Do you think I’ll need a tummy tuck too with my BBL?
For this patient, BBL offers no improvement.

A BBL will not offer any improvement to your abdomen. A tummy tuck will remove loose skin and can remove fat as long as it is not deep and under the muscle.  You would need to be examined. The picture is not adequate.
What is the right order to have surgeries, time space between each, and which can be done together

You could have all the surgeries at one time and you do not need multiple recovery periods. The cost would be much lower doing all at the same time. I frequently perform breast surgery, tummy tucks and liposuction at the same time. I am not a fan of BBL. Nationally there are significant complications. I have seen poor results from the technique involving the buttocks and deformity from where the fat was taken.

The main thing I notice from your pictures is marked asymmetry of the breast implants, which is much more obvious than a bulge in your abdomen. The abdominal bulge is either separation of the rectus muscles or intraabdominal fat. Muscle repair is usually easy to perform at the time of a tummy tuck. If it is the problem, it could still be repaired. For intraabdominal fat, you would need to loss weight, not have anymore surgery

If your implants are under the muscle, removal should result in your original appearance. The muscle should be support the weight of the implant and not the skin. If the implants are on top of the muscle your skin will stretch the longer the implants are present.

Without the opportunity to examine you, it is difficult to give a strong recommendation. I would encourage vigorous massage for a few months, which is what is commonly done for tight scars. Hopefully that will work. If if doesnot, you may have had too much fat removed and may require some fat grafting to separate the skin from the muscle. Good luck.

No size of implant will correct your low nipple position. You need a lift to improve the nipple position. I would suggest you see a surgeon who uses an imager to preview what you will look like without a lift. You may be satisfied with the volume and not care about the low nipple position.

Your right breast implant is too low. Without examining you and reviewing an op note, I can not tell if there is no muscle over the bottom of the implant. Muscle repair may be an option. Fat grafting would be of no benefit and has risks associate with it. It appears that the right pocket needs to be closed inferiorly. I would reposition existing tissue, but some surgeons would use a mesh product.

It sounds like you may have a capsular contracture. A normal result should be soft. Shaped implants are firmer than round implants and carry additional complications. Allergan shaped implants have been withdrawn from the market due to concerns of a rare cancer, anaplastic large cell lymphoma. There will be minimal to no effect on the medial thighs with a lower body lift. There can be a noticeable improvement of the anterior and lateral thighs with a body lift. A medial thigh lift can be performed at the same time or later. An incision from the groin to knee will give the best contour, but leaves a long scar. An excision in the groin crease can help hide the scar, but typically only tightens 25% compared to the long incision.

I have performed liposuction only breast reductions on over 1300 women. The average reduction is 2 cup sizes. I have used liposuction on 20 women with implants to successfully reduce the size. This is possible is the implants are under the muscle. The average improvement is nipple position is 2 inches. If you need more than that a lift can be performed at the same time.

Since you are extremely stressed, I agree with your decision to change to smooth implants. There is no good scientific data to support capsulectomy, but it is reasonable since it is in the capsule layer where ALCL develops.Any competent plastic surgeon can perform a capsulectomy. There is a small risk of bleeding and you should have a drain placed at the time of surgery.

The ideal implant is a high profile saline implant. A silicone implant should feel better. You could switch to a moderate or moderate plus profile implant, which will have a more natural shape with less or an over-inflated, round look. There is no reason to wait longer. Your current surgeon should be the least expensive option. There should be virtually no pain and they should look good immediately.

Without seeing photos it is hard to tell how much smaller you will be and what type of lift would be appropriate. Unless you have a revision of the pocket, the implant will still move too far to the side. Going down 25 cc is insignificant. The lift will tighten the skin, but not improve the pocket. The lift will also make you smaller.
