A conservative shave of your Adam’s apple could give you a more feminine neck. Would have to consider that a shape of the Adams apple would only take care of the midline projection at the very top of the thyroid cartilage.

I am sorry you are having this issue. Developing wrinkles on the nose may be a sign of normal aging. If you are concerned that the support of your nose is change, then you should be evaluated by and otolaryngologist .

Does deviated nasal septum or nasal obstruction can cause tinnitus(two ears) at night .

It is very unlikely your tinnitus is related to your deviated septum. Nasal obstruction is multi-factorial and could conceivably be linked with tinnitus.
You should be evaluated by an otolaryngologist for these concerns.

Correction of your dorsal hump, likely saddle nose deformity and septal perforation is a complex problem that may require multiple phases to the surgery. Either way, you need better support for your nasal tip and mid vault. The correction of the perforation may require tissue flaps from the remainder of your septum or turbinate tissue flaps.

Great questions! Rhinoplasty should not produce any significant risk due to your underlying condition. The nose has very robust blood supply from many areas and the columella incision will no propose an additional risks.

You have beautiful framework to your nose, but I do see your concern.
Clinical exam and sometimes a CT scan can be beneficial to really understand the anatomy of your nasal bones, especially if you’ve had nasal trauma.
Overall, a rhinoplasty through a closed or open approach would address your concerns.

In order to fully evaluate the complexity of fixing the septum the entire septum should be evaluated in clinic. Based on the external appearance of your nose your septum is unlike to be extremely twisted. At photo review, your case should be straightforward and produce beautiful results.
