This looks like a keloid scar. Some people more apt to get those. 5fu and radio frequency should be able to flatten it easily and is cost effective.

You need to find out what filler was used .
There are a lot of ways to dissolve fillers
5fu with decadron and collegenase works great . There are many physicians that know how to do that .

Unfortunately in dark skin peels can cause hyper pigmentation if that peel is too harsh. I would recommend you use a product called lytera It takes longer to work but does not have the side effects of other lighting agents.
