Trouble Breathing and Nasal Voice After Rhinoplasty, Is This Normal?
5 weeks after nasal surgery
Jul 6, 2011
Hey Gothamgirl – thanks for your question – nose surgery is the operation I do most and invariably with routine post-op care each week for the first 3 weeks, my patients are breathing better than they have ever breathed their whole life. Your problem could be one of 3 main problems – the most likely is that you have retained secretions and or scabs in the nose which have not been cleaned and cleared out – the second likely reason for the nasal voice and breathing issue is that during…
Rhinoplasty on Bulbous Nose, Can I Achieve Desired Results?
nasal tip
Jun 30, 2011
Dear babybush101 – thanks for your note – I would agree with most that the definition achieved in your tip will be a combination of skin thinning and cartilage restructuring. I would not suggest rotating the tip of the nose more but simply increasing definition as described above. A natural look is vital to a nice nose – be conservative. PK
Botox Emergency Bad Reaction!!! Help! Will Botox Divots Go Away Around my Eye??
BOTOX divots
Jun 30, 2011
Hey Suzette501 – I will agree with the others on panel – Botox in my hands has never in 18 years created such a problem – again ensure the injector is qualified and the product used was indeed BOTOX. Each vial used has a lot number and you should ask for the lot number to confirm for yourself. The longevity of the product as you know is 3-6 months – if you have not seen resolution of the divots I would at that time consider filling the area for better symmetry. Good…
What to do about the hole in my nose?
septal perforation and nasal collapse
Jun 2, 2011
Dear Jayha – thanks for your question – you have a very difficult problem but one that is reparable based on your description. The septal perforation repair operation and the reconstructive effort for the falling nose second to the perforation is the most difficult operation that we as Facial Plastic Surgeons do – finding a doctor with a good number of these operations under his or her belt is important. I would make no consideration for enlarging the perforation as this is…
Restylane For Under Eye Hollows After Weight Loss
lower eyelid restylane
Jun 2, 2011
Dear Mermaid911- great question – using a filler in this area is definitely of benefit – it address the true reason for the aged appearance – a loss in volume – I don't think that consideration for a lower eyelid lift in your situation is a good idea – unless a fat respositioning maneuver is considered – this would of course require that you had fullness immediately under the lid which based on your photos I don't see. The Restylane or Juviderm is my only suggestion for filler…
Looking for Rhinoplasy Surgeon in Connecticut or Surrounding States
rhinoplasty surgeon
Jun 2, 2011
Hey KaylaD – I am glad to hear that you are looking around to find the right doctor for your concern – while I agree that price should not be the only criteria for choosing a doctor to do your nose I can realistically understand why it is part of your equation. Choosing a doctor with lots of nasal experience may lead you to look for a Facial Plastic and Reconstructive Surgeon – one who definitely understands both form (the outside shape of the nose) and function (the breathing job that…
Very Self Conscious of Nose Since Rhinoplasty.
flat bridge and tip
Jun 2, 2011
Hey Dels – sorry you are not happy with your nasal surgery – it would seem that the middle part of your nose has either collapsed or has been over resected. The tip is rotating up as a result of the middle nose change – at a year you are probably not going to see much improvement. I would suggest that you find a different surgeon to address your concern – if you feel comfortable returning to your primary then by all means – I would be certain that he is able to see and…
When is a Revision Rhinoplasty Appropriate? As a Last Resort or Asap?
infected nose and revision surgery
Jun 2, 2011
Hey NYCgirl2011 – sorry to hear you are having troubles – I would hope by now that your primary PS has cultured the nose and is directing the antibiotic towards the bacteria present – if you are getting less tender and less swollen it suggests that the infection is effectively being treated – you can continue this path as long as you continue to progress. If you stop improving then a simple intranasal approach to remove the Gortex primarily may be indicated. I would suggest that…
Strong Disagreement with Plastic Surgeon’s TCA Post Peel Advice
8% TCA peel
Mar 22, 2011
Dear Soniak – I would agree that the 8% TCA is not overly aggressive at all – and I would suggest to you that the routine outlined by your practitioner is not unreasonable.  The Retin-A and the Hydroquinone are prepping the skin for the peel.  My instructions if I were to offer such a skin routine would including stopping these two products 2 days prior and not starting again for 2 days after the peel.  The weekly application (for 2 applications) of the 8% TCA is…
Large Internal Scabs 2 Years Post Septoplasty
scabs in the nose 2 years after septoplasty
Mar 22, 2011
Dear Swanster – I would agree with the previous comments that something is not quite right – scabbing 2 years post septoplasty suggests that either a chronic rhinitis or infection is present which may require a culture and an ointment like Bactroban or it may suggest a hole in the center divider or septum.  Either way your nose should be evaluated by your local ENT/Facial Plastic Surgeon. Should you have a perforation, know that repair is very possible depending on the size of the…
Broke Nose 3/2/11 – How Soon Can I Have Rhinoplasty?
fractured nose and rhinoplasty
Mar 22, 2011
Dear Columbia – thanks for your question – I think that most would agree that waiting for the majority of fracture healing to be complete (4-6 weeks) prior to a formal rhinoplasty (nose job) is in your best interest.  The healing of the nose is an unpredictable process and this is one reason that the Rhinoplasty is the most difficult operation we as Plastic and Facial Plastic Surgeons perform. Once the injury from the fracture is healed, the rhinoplasty which is in and of…
Bulbous, Uneven and Bent Nose After Rhinoplasty
bent nose
Mar 22, 2011
Dear Dexta – thanks for your question and sorry that you are having outcomes trouble – I am in agreement with the panel who suggest that your surgery did not go as planned – it does appear that the left side of the nose has lost support and structure – this may actually worsen as the healing contracture takes place.  Not having any pre-operative pictures I am concerned that a pre-existing cartilage or support issue is now made much worse by whatever was done so far. You need to wait the…
Will Silicon Implant in Chin and Jawline Need Replaced?
silicon chin implant
Mar 22, 2011
Dear Ms LizLiz – thanks for your question – the solid silicone implant is a very safe and stable product – with good implant technique and good post-operative care the implant will be incorporated by your body and provide the profile improvement that you are looking for.  While the implant does not change it can create a minor amount of boney resorption on the jaw.   As you age the bone of the jaw will change and if you loose your teeth it may change even more.  This is…
Can Stitches Still be in my Nose 5 Years After Rhinoplasty?
stitches in the nose after rhinoplasty
Mar 22, 2011
Hey Kamila – the bump you are feeling in the nose is most likely the leading edge of one of the cartilages found in the tip of the nose – when this area is trimmed the nasal tip is thinned and if the incision site is not closed it is possible for this structure to prolapse or fall.  As the nose matures this may also happen and can even become a bit worse until it levels or completes the settle process.  I agree with others who indicate that it is not terribly uncommon for an inside…
Infection Surrounding Chin Implant?
infected implant
Mar 22, 2011
Dear San Diego – by now I am sure that you have sought attention for the infection in the chin implant area – there is not question that the site is/was infected and immediate attention is required – Keflex is not my favored choice for an antibiotic in this area – Cipro might be a better choice while a culture is taken and specific treatment can be directed.  Hope all works out well and you can decide if another implant will be placed in the site.
