Can you breathe at all with nasal packing in after a rhinoplasty?
No packing after rhinoplasty
Oct 27, 2018
Some surgeons routinely employ the use of packing, or plastic nasal stents, after surgery. Packing does prevent one from breathing, certainly. Some patients dont find this all that difficult, but others are quite uncomfortable. Personally, I dont routinely use packing nor splints, and find that the healing is just fine.

Imperfections after rhinoplasty
Sep 2, 2018
You are correct, that there are imperfections visible on your nose now. However, you look great! The reality is that there are always imperfections after rhinoplasty. There is no way to avoid them completely. Typically, they are visible when the nose is closely scrutinized. All human beings have facial imperfections. Other people generally dont pay attention to small details. Certainly, it is completely appropriate to discuss your concerns with your surgeon. Sometimes, it makes sense to try ...
Skin graft from MOHS surgery keeps growing long dark hair. Is repeated treatment to be expec
MOHS surgery on the nose
Aug 31, 2018
Skin grafts are occasionally a reasonable option to treat MOHS defects on the nose. When possible, we choose donor sites that match the skin characteristics of the nose optimally. No donor site is perfect, but we often use the forehead as a donor site. This location is well known to match quite well with our experience performing paramedic forehead flaps. It turns out that taking a graft high on the forehead, along the hairline (but not IN the hairline) can work really well. Perhaps the h...
What is the main difference of an open vs closed rhinoplasty? Is recovery time different for
Open vs closed rhinoplasty
Aug 17, 2018
This is a common question, and I think there may be some confusing information online. The key difference is simply how one chooses to gain access to the structures of the nose. Does the surgeon make incisions only on the inside, and thus work through smaller openings? Or, does he or she extend those same incisions around the front of the columella (the little structure between the two nostrils) in order to get a wider field of access? The advantages of the latter are pretty obvious - g...

scar tissue in nasal tip
Apr 13, 2018
There is always a certain amount of scarring that develops after rhinoplasty. The amount differs from patient to patient, and from case to case. After a year and a half, while there still may be some improvement over time, but it will not be much. Rather than focusing solely on the presence or absence of scar tissue, and the possible removal, I would instead encourage a discussion with your surgeons about options to achieve a result closer to your objective of a nice appearance to the tip. T...
Revision rhino: I was wondering the resorption rates of DCF when used to augment the bridge.
Finely diced cartilage for dorsal augmentation
Mar 19, 2018
We have been very happy with the longterm outcomes of finely diced cartilage dorsal augmentation. We find it to be more reliable, overall, than en bloc cartilage onlay grafts. In reality, there is no perfect rhinoplasty graft - and so there is variation in outcomes.
Is my dream nose even attainable?
Can you obtain your dream result?
Feb 18, 2018
The best way to think about rhinoplasty is that it can help to make your nose look better than it looks today. Obtaining a perfect nose, or your "dream nose" is not typically possible. I see no reason why you couldn't get nice improvement.
Revision Septorhinoplasty with Upper Blepharoplasty: same time or separate?
Combining rhinoplasty with eyelid surgery
Feb 7, 2018
This is a pretty frequent question - where many patients will ask, "hey, while you are in there...."  It can make sense to do so. The advantage is that you really only have to go through a healing phase once instead of twice, and often there is a bit of a reduction in the out of pocket expense as well, since procedures are combined. So, these can be good reasons to combine procedures. Medically, it is usually fine, but there can be important exceptions to this, in which your surgeon feels i...
Will rhinoplasty be right for me?
Rhinoplasty and breathing
Jan 30, 2018
Your pictures indicate that you are likely a very good candidate for a rhinoplasty. Combining aesthetic changes with work to improve breathing is really very common. I am especially interested in your hope to look more feminine "while still looking like me." This is the great challenge and one of the wonderful things about rhinoplasty! My experience has been that this is the best way to approach the setting of goals for both the patient and surgeon - and typically patients are both relieved ...
Do you think my nose needs a 3rd rhinoplasty?
third revision rhinoplasty?
Jan 29, 2018
There is virtually never a case in which someone "needs" a rhinoplasty! What matters is how you feel. Also, perfection is impossible - so you have to think carefully about whether or not it makes sense to go for a third surgery. Risks increase with additional procedures. Your tip could use some more projection, but the decision will depend on many factors, including how scarred and distensible your skin soft tissue envelope is. Please be cautious, and also be kind to yourself (many people a...
Are drains placed after a face and neck lift? And for how long?
Drains after facelift
Jan 29, 2018
This is a great question. One reason to place drains is to limit the consequences of the most common complication in facelifting - hematoma. This is when a collection of fluid develops under the skin. In theory, placing drains "pre empts" this by allowing any accumulating fluid (serum, or blood) to be diverted out through the drains. Interestingly, there have been studies showing that placing drains may not lesson the risk of developing a hematoma that requires treatment. Personally, I ...
Do I need to wait to have surgery if I am 18? I have a deviated septum.
Septoplasty and rhinoplasty in teenagers
Jan 15, 2018
We generally feel that proceeding with nasal surgery is safe at your age. A rule of thumb often used is that a female patient should be at least 15 years of age, while a male patient should be at least 17 years of age.In reality, there can be exceptions - in either direction. Just keep in mind that the decision to pursue nasal surgery is a serious one. So it may make sense to get multiple opinions.
What is the difference between a mini lift and a full lift?
Minilift vs full facelift
Jan 8, 2018
This is a pretty important question to ask your surgeon. Basically, the two procedures differ in the degree to which the soft tissues are elevated, undermined, and advanced. In addition, often a "mini lift" avoids much if any release and elevation of the skin and soft tissues of the neck.In my experience, it is most often the case that a full face and neck lift is the best procedure. There are exceptions, especially for patients who have more youthful skin and soft tissue, and do not have m...
Lower face and neck lift in March 2017. Skin laxity under chin. Division of platysmal bands.
When to do a revision facelift or neck lift? What type of revision is best?
Dec 17, 2017
The laxity that can persist, or return, after a lower facelift and neck lift can be tricky. Personally, I am more apt to do a more invasive revision, but it really depends upon a number of factors. These include, but are not limited, to your own risk tolerance and tolerance for additional downtime and healing, the characteristics of the deformity, how it looks from different viewing angles, and more. I am also skeptical of the expected duration of minimally invasive treatments like thread li...

revision of ala for asymmetry
Dec 14, 2017
Some asymmetry of the ala after they are reduced is very common. Typically, it is really only seen when looking from below, holding a mirror (or camera) low to see it from an angle that others rarely see. This is important, because if one is happy with the changes from the frontal view, and profile, it may be best to simply accept some assymetry as an acceptable tradeoff of having surgery. Othertimes, there are additional reasons to intervene and it can make sense to do so. For now, consider...
