The changes you describe are quite standard and would not be considered drastic by any means. The desired outcome of a procedure is a very personal choice.

What one person thinks is drastic might be considered conservative by another. Most surgeries are easy to accomplish in a single procedure.

That’s not to say that a small percentage of patients will request revision surgery. However, planning two separate stages is not common practice.

Rhinoplasty is often a procedure of small changes to enhance the characteristics of the nose, though sometimes ‘drastic’ changes are needed to reach the goal of the procedure.

Even with very large hump reduction, correction of a severely twisted nose, thinning a very full tip, rhinoplasty is a single procedure tailored to the task at hand, rather than a ‘staged’ procedure. Whatever changes your nose requires to achieve the results you are seeking are best accomplished in as few operations as possible! In most cases this is ONE operation, but every surgeon who does nose surgery has occasional patients who request or require additional surgery.

PLANNING in advance for more than one procedure is generally NOT recommended, though I sometimes will do all of the changes that a patient requests EXCEPT for the Weir excisions (at the side and base of each nostril) to reduce lower nasal width and nostril size. The latter can easily be performed as a separate procedure under local anesthesia if either the surgeon or the patient (or both) are not sure if the decrease in nostril size or lower nasal width is really necessary after all of the other requested changes in the rest of the nose.

These require that all the work be done when the patient is asleep (or sedated) and the nasal skin is elevated from the underlying support cartilages and bony structures. Patients sometimes worry about what happens to all of the “extra” skin after the nasal hump is removed.

Although you are considering multiple changes in your nose, this is not “drastic” surgery. An experienced nose job surgeon will bring all nasal structures into proportion – this can and should be done in one operation.

When I first started in practice 35 years ago may surgeons did aggressive rhinoplasties, removing a lot of tissue, which resulted in noses with pinched tips and scooped-out profiles.

Drastic nose job – More Than One Surgery Required?

What you describe is the hallmark of ethnic nose. This is the most challenging surgery requiring surgeon experienced in this type of noses. This is achieved in one surgery.

We have never told patient that two or more surgeries needed. Once, what is needed is done, then all that is needed is occasional steroid injection during the immediate post operative period to reduce the scarring and swelling.
