Fixing a deviated septum is a pretty routine part of my rhinoplasty procedure, especially when it helps me to get the cartilage grafts that I need to finish the job. If the aesthetics of your nose bother you, it is best to get the rhinoplasty done at the same time as the septoplasty so that that cartilage material doesn’t go to waste.

The two main things that I evaluate on profile are the chin projection and the nose. Your chin has adequate projection, but your nose would benefit from refinement by lowering the bridge and raising the tip slightly, while keeping it very natural appearing. This can all be done with closed rhinoplasty.

Sometimes I still do it for patients who are having trouble losing the extra weight just to help get their body transformation process started since it is quite motivating to see these changes and then stick with a diet plan better. I do not recommend putting permanent filler like silicone in the nose. It can cause problems that will be life-long. In the right hands and put in the right locations, however, hyaluronic acid fillers can do wonders to improve the nasal profile (side view) and straighten the front view of the nose. Juvederm Voluma can last up to two years so, most likely, you don’t need to do any removal process prior to chin augmentation with an implant. Of course, before you do the chin augmentation surgery, you should be sure to tell your plastic surgeon that you had filler placed so that he can make sure that there are no remaining bumps on his examination and review your expectations regarding how far you want to increase the chin projection.

Liposuction and excision of extra breast tissue accomplish very different things. I find that excision is often necessary in cases of a true roll of skin that is loose. The downside is that it leaves a longer scar, but it is a very powerful procedure. Liposuction can gain a very good result alone in the right patient who has tight skin in the area. See a board certified plastic surgeon to determine which procedure is better for you.

I would not focus on closed vs open rhinoplasty. You can be helped with closed rhinoplasty as well to create more of a natural flow between the different areas of the nose and address your concerns. Speak further with a nose expert to determine your surgical goals and begin to understand what can and cannot be accomplished with this surgery the second time around.

Your concern is very common so nothing to fear. You have not posted photos, but the issue that you are describing is often due to lack of support to the nasal tip. That being said, it is also normal for the tip to move a bit (or at least give the appearance of movement) when we smile due to the activity of the muscles that surround the nose. Sorry to hear about your breathing issue after rhinoplasty. This can occur due to a number of reasons that range from persistent swelling and scar tissue to graft displacement and untreated deviated septum. You should continue to see your surgeon but, if you are not getting the relief or the answers that you need after a few tries, you might want to consider a second opinion. You would need a good in person physical exam to figure out what is going on in this case. Best of luck!

Hi Isabella, you do not appear to be an ideal candidate for a tummy tuck at this time. It would be best for you to continue to try to lose weight so that the fat behind the tummy muscle shrinks more and leaves you with mostly loose skin in front of the muscle. This way you will be able to achieve the flattest abdomen possible!

For the long term, closed rhinoplasty would be the best option to take down the bump. Filler will allow you to disguise the hump by raising the root of the nose, but won’t make the bump go away.
