On the side view it does not look upturned. I think what you are seeing is flared nostrils and a hanging columella. The treatment would then be an alar base reduction and cutting back the caudal end of the septum. Your best bet is to see a surgeon in person and have computer imaging performed to see the effect of those procedures. You should also broach the subject of surgery to realign your tip cartilages and decide if you want that done at the same surgery.

With the limited information in your post it's impossible to say if you are even a candidate for the procedure. However, Apple Valley is less than 2 hours by car from Pasadena and San Bernadino and there must be 50 surgeons in that geographical area and between who could and are qualified to do the surgery so I don't see why you would need to fly anywhere.

This looks like you had a cheek bone or ZMC tripod fracture and the bone shifted/rotated downward due to the pull of the masseter chewing muscle. You will need a CT scan to see if that is the case. If so the corrective surgery would require refracturing the bone along the initial fracture lines with osteotomes, moving the bone back into position and then fixing it in place. Your photos show a lot of extra skin circumferentially and a tummy tuck that does not go around to the back is not going to address this. If you end the tummy tuck incision on the front you will have a large pleat of skin just beyond the scar. If you have a belt lipectomy you can remove the skin on the back and move a flap of fat into the buttocks to augment and lift them. This looks like scarring with retraction of the deeper layers. If that is the case scarring in the middle layer needs to be divided, grafting of the deep layer (usually from the palate) is required to replace the loss of retraction and the outer layer would then be augmented by sliding cheek skin upward. There are probably as many ways to take the measurements as there are surgeons doing it. Breast diameter is the least reliable because most surgeons use a flat 2 dimensional ruler to measure the width of a 3 dimensional dome with forward projection. Onlay cartilage grafts for depression are usually placed over depressions in the nasal bone. This is over the alar-upper lateral cartilage junction. If this is not where the graft was placed it either slipped out of position or this is scar. Your surgeon is in the best position to make that differentiation. This looks like early lipedema especially on the right leg. If that is the case liposuction is the recommended treatment and early treatment is better than late treatment. Also, liposuction is palliative for lipedema as it cannot be cured.

It looks like your alar base width is mostly due to thick nostril rims. Thinning the rims can give you 2 to 4mm on each side (4 to 8 mm total) thereby narrowing the base and increasing nostril size at the same time. You should also think about whether you want the retruded columella addressed.

Your photos show a wide bridge, a drooping depressed tip, a parentheses shaped tip, a cleft between the columellar cartilages and presumably a wide alar base. If you do it closed you will need a columellar strut. If it's done open you would have the option of a septal batton and more control of the end result. It can't be made exactly the same but I think a small alar base and tip forward projection reduction along with standard tip cartilage trimming will get you closer to it. If you don't trim the tip cartilage you will retain the bulbous-ness but will look less like the results you say you want.
Are my breasts tuberous? And if so, can they be fixed with regular augmentation with silicone implants?

I would not consider either one tuberous. They are asymmetric in size, shape and droop. Therefore I don't think just putting in an implant on the left will suffice. The left is more of a round shape and will continue to be so after implant placement. The left crease also looks higher than the right one. It would be easier to make the right look more like the left containing an implant than make the left look more like the right. You will require a shape changing lift on the right to do that...

Not knowing what you looked like before surgery or how much improvement you had from it I am sorry to say it looks like you are going to need a revision surgery. In these photos the tip is asymmetric as are the nasal bones. It is not clear whether the best option is tipplasty and camouflage of the left nasal bone or tipplasty and bone surgery. I suspect it will be the latter.

For privacy it's better if you crop your submitted photos. I suspect you will need a small lift in addition to an implant in order to achieve the upper fullness you are looking for. From your photos it's not clear that you are a good candidate for a tummy tuck. I highly doubt you would end up without a vertical scar unless the horizontal scar is placed too high. I am not sure that a hip to hip scar is worth the amount of improvement you will have from a tummy tuck.

When I read your post I didn't believe it but looking at the photo it does look 6cm wide at the alar base. The desired size is usually equal to the distance between the inner and outer corners of each eye (the intercanthal distance normally is 3.2cm). That would require an alar base reduction of 1.4cm from each nostril rim. That is more than usual so I suspect you will also need some thinning of the nostril rims in order to decrease the resection size to avoid a pinched nostril appearance.
