It is difficult to make recommendations on only one photo, also it’s important to fully understand what you are looking for in a rhinoplasty. You appear to have a hump and an over-projecting tip (stick out too far) as you suggested. You also appear to have strong tip cartilages since we can see them through the skin. To properly plan a revision rhinoplasty you need a dialogue with your surgeon. First you need to share the same aesthetic goals. Second, your surgeon has to make sure that these goals can be met, given your structure, airway and skin thickness. The small bumps on your nose are most likely due to a natural stepoff from the nasal bones to the cartilage below, called the upper lateral cartilage. Rasping the bone will not alter that junction.
