You have a nasal deviation with likely underlying septal curvature. You also have thick sebaceous skin. These are the challenges in doing your rhinoplasty. A nice open septorhinoplasty Can be achieved though with reasonable expectations in an experienced surgeons hands. Congrats on the good health and exercise and best of luck!

Quarternary revision rhinoplasty

Each time you have surgery scar tissue is laid down which it more resistant to long term change. That being said, you’ve had a couple septoplasties and a sinus surgery but you did not list a rhinoplasty. To correct the deviation a complete rhinoplasty would be needed with cartilage grafting holding the septum in place. Essentially, your entire nose needs to be taken apart and put back together with appropriate cartilage support to achieve an improvement in the straightness of it. Best
