Nose is too large and wide for my face. Possible rhinoplasty and chin augmentation?
Rhinoplasty and chin related surgery
Apr 19, 2019
Thank you for your question!  I would state that a conservative tip rhinoplasty with alar base reduction, tip refinement, cephalic margin narrowing and contouring would provide the best results with possible subtle tip augmentation.  I would not recommend a genioplasty or chin augmentation as your profile showcases an appropriate chin position.  Any chin droop during smiling would be a dynamic issue not easily or possibly corrected by a surgical option.  Sometimes conservative Botox will…
Can I benefit from lip lift and rhinoplasty? Which one should I do first?
Lip lift and rhinoplasty
Apr 19, 2019
Based on your conservative desires a simultaneous option is very reasonable to pursue for both base narrowing and a lip lift procedure.  In this circumstance an incision can even be combined to solve both issues at once.  You may also consider a nasal tip augmentation to provide more length and projection of your nasal tip as well for the maximal outcome and result.  Hope that helps!!
Am I too young to get any form of fillers? I’m 18 years old and I hate my smile lines.
Filler to smile lines
Apr 19, 2019
Age is not necessarily important as the issue at hand which could be improved and corrected in your circumstance.  The wonderful thing about filler material is that it is temporary and safe in the experienced hands that has little downside besides short term bruising and swelling.  Juvederm XC is a great option as it is smooth, stable over 9-12 months, soft, natural and will absorb naturally over time.  There are other options based on injector preference but that may be an excellent opt…
Is 4 months before my wedding too soon for rhinoplasty?
Rhinoplasty prior to wedding
Apr 18, 2019
This is tough because of the two issues being as important as they clearly are to you.  In general terms 4 months is a very reasonable period of time to ensure appropriate healing from a rhinoplasty for near term results but the long term will not be present for a full one year.  All surgery has drawbacks and you could have some natural but unpredictable swelling that is not favorable to you or the eye of the camera and this would likely create more stress and concern leading into the w…
What procedure could correct the depression in my nasal tip? I am happy with all other aspects of
Supratip depression following rhinoplasty
Apr 18, 2019
Revision rhinoplasty for a defect such as this may be aggressive overkill.  You can absolutely consider non surgical options such as Juvederm for a mid length improvement of approximately 1 – 2 years.  The nose holds filler material longer than other mobile areas such as the lips and nasolabial folds.  In my experience this has been a great option for those that have arrived seeking second opinions due to sub optimal rhinoplasty results.  You need to be very careful, and very judicious in t…
Can the bridge of my nose be straightened without making the nose look feminine?
Straight nose with dorsal hump profile
Apr 18, 2019
Absolutely!  Based on the photo you submitted a conservative rhinoplasty would provide a very appropriate and balanced look while maintaining your masculinity but creating maximal refinement.  This would be a very straightforward operation in experienced hands!!
Can anyone show me where exactly botox is administered when only treating glabella area?
Glabella Botox
Apr 18, 2019
The shortest answer to your question is to look at the most medial portion of your eyebrow.  Just above this approximately 1cm is the insertion of the corrugator muscle and is most appropriately injected to diminish its action in this location.  With that being said it is common to perform 5 injections in a V shape across this area in a targeted fashion to access the corrugator and procerus muscles to diminish glabellar lines.  Technique varies but this is the closest answer to what I …
25 y/o female with jowls. What can be done to fix this?
Early jowling
Apr 8, 2019
The best option is to consider either owning the heart shape to your face and chin distinction which is attractive btw….or consider a prejowl option for filler.  So imagine in the area where you pull to create the V line shape to fill that area.  Now the downside is it will make your face look fuller.  The shape will be more appealing but you are concealing reality by adding even more volume.  It will look better but some women will worry that it makes their face appear more round.  That…
Can a smoker reverse facial aging if they stop smoking?
Aging after quitting tobacco
Apr 8, 2019
The amount of cellular regeneration that occurs after quitting smoking and alcohol use can be remarkable.  Our immune system only has so many resources and if they are constantly battling the inflammatory condition of carcinogens there is little available for skin protection.  You would see your skin hydration improve and fine lines and wrinkles recede.  The sallow color to your skin would improve as well.  The volumization around your eyes is unlikely to change but areas around it will thu…

Odor following rhinoplasty
Mar 7, 2019
Nothing to fear!!  This is all well within normal as there is commonly increased or altered drainage issues from the nose in the early postoperative period.  The only concern would typically fall 1 – 2 weeks following surgery if dissolvable (chromic) sutures were utilized as these sometimes can become colonized and create a localized infection.  But even in that worst case scenario it is easily treated with a short course of antibiotics and all will be well.  You seem to merely be exp…
Why is my right nostril clear 3 days post opp?
Breathing after rhinoplasty
Mar 7, 2019
Despite the fact that your nose is a singular structure the healing process never truly occurs perfectly symmetrically.  The other issue that is routinely common is that drainage, congestion and mucous flow and production do and will change following surgery.  However, you are so early postoperatively you should confidently know that there is nothing to worry about and this is all well within normal limits.  A gentle saline spray to the nose or a simple aquaphor treatment for comfort and mo…

Implant following surgery
Mar 7, 2019
A simple CT sinus imaging study will reveal the plates and screws and obtaining the operative notes will help determine the company brand so your surgeon can match the plating system and the instruments appropriately if the hardware requires removal.  The K wire will 100% show up on the imaging. If there is nothing rewarding gained from the CT imaging study in the past I have ordered an MRI to evaluate other non-metallic implant materials such as gortex or silastic to determine the un…
I’m about to turn 43 and not happy with my neck. Am I too young for a neck lift?
Neck lift expectations
Feb 27, 2019
Thank you for your question!  Based on the photos I would argue that you may benefit from a platysmaplasty (anterior muscle tightening) and submental/lateral liposuction only.  This would prevent the need for a formal neck lift or facelift while still achieving dynamic results with significant improvement.  This is performed through an anterior incision under the chin only with far less downside and downtime in comparison to a traditional neck or facelift.
Follow-up question: Will my smile gradually improve after getting cartilage grafts salvaged from
Smile after rhinoplasty
Feb 27, 2019
Yes!!  It should all return to your normal baseline.  Columellar strut grafts can sometimes impact the depressor septae muscle underneath your nose impacting your smile in the short term.  Once the swelling subsides then you should have complete resolution of the issue but it typically takes 3-4 weeks.  Overall there should be no long term impact unless the goal was to prevent the tip of your nose from drooping and division of that muscle was a planned maneuver.  Give it time and all shou…
Is a bump normal after onlay graft rhinoplasty or is it swelling and will disappear?
Bump after onlay grafting
Feb 19, 2019
There are a great many things that will happen in this situation.  My best recommendation is to not fear and have patience!  In a revisional surgery scenario, especially with onlay cartilage grafting, time is the key feature.  You will see dramatic changes in swelling, tissue density, cartilage settling, and even some minor cartilage resorption over the course of the first several months.  As surgeons we have a short window to perceive our expected long term results during surgery but the…
