I agree with your surgeon that you could benefit from more chin projection with a chin implant. Kybella for sub mental fat removal, or liposuction, could be performed simultaneously as well. All of these procedures including rhinoplasty can be simulated with imaging software. Please note that chin augmentation is routinely performed at the same time as rhinoplasty, and in fact adding sub mental liposuction at the same time could be performed. Best of luck to you!

Thanks for the question and photo. I believe your brows are in good position and have very little asymmetry. The redundant upper eyelid skin can easily be dealt with skin only upper blepharoplasty. This is a procedure that I perform in the office under local and only takes about 20 minutes. You will have some social downtime from bruising, usually only about a week.

The fatty tissue in your neck is generally treated with liposuction, often in the office under local anesthesia. Jowls can be lifted with surgery, tightened with non invasive techniques like Ultherapy, or camouflaged with filler in the area in front of the jowl known as the pre-jowl sulcus.

It's not an age issue when we are discussing facelifts with patients. There are 40 year olds that are appropriate candidates, and 80 year olds that aren't. An individuals candidacy relates to how much laxity exists in the cheek and neck area. The more laxity, the more we can lift that tissue. Based on your photos, you have very little apparent laxity in these areas.

There are no documented research papers on these various techniques, so my suggestion is to pick a surgeon with a great reputation and one that you feel comfortable with.

The first thing that stands out to me is your lower eyelids. Was filler placed in this area, i.e. the orbital hollow? It seems that you are very puffy there, perhaps from the filler, or it might be just your anatomy, hard to tell from the photos. A direct browlift is certainly an option for patients. An incision is made along the upper brow and skin is removed. The incision is closed and this allows the brow to be lifted.  It can be done in the office under local anesthesia. Good luck!

You don't have signs of significant jowling. In your age group I find Botox and fillers are common procedures to battle facial aging. As we age, we lose volume, so your volume requirements will be higher through the years. Be sure to find a Board Certified Facial Plastic Surgeon to do your injections, as they ONLY work on Faces.

Aging and health problems both lead to weight loss and facial aging. In fact volume loss in the face has more to do with facial aging than gravity. Sounds like you need volume restored. The short answer is yes, just the tip of the nose can be reduced during rhinoplasty, but this might not give you the best result. Your picture shows moderately wide nasal bones and if you undergo just tip rhinoplasty, these wide nasal bones will be very evident.
