It appears that there are significant remaining wrinkles of the lower eyelid skin. I would suggest ablative laser resurfacing to the lower eyelid skin. This procedure significantly increases collagen in the skin and significantly reduces wrinkles. I would suggest botox before the procedure as it relaxes that skin and reduces wrinkles during the healing phase which is essential for an optimal outcome. 
3 weeks post op rhinoplasty, white stitch poking through outside of nose. Is this normal?

It is difficult to answer your question without seeing photos of your nose. The cosmetic appearance of the nose is dependent on all parts of the nose coming together in a balanced way. The nasal structure is divided into thirds, the upper , middle and lower third. It is rare that a single change to one area is the only surgical improvement that would be necessary. Your scar appears to be depressed which creates a shadow and makes this more visible. A HA filler can often improve this by making the contour flat. However all scars are unique and may or may not fill uniformly. As always, I suggest that you seek an evaluation by an experienced clinician that has treated many scars and has multiple modalities at their practice so that you can achieve the optimal outcome.

If you have never  had a rhinoplasty then this would not be characterized as a polybeek deformity.  The primary issue with your nose is lack of tip support and projection. You may require a slight reduction of the dorsum however the major priority is to improve the structural support of the tip. This will create a stable long-term aesthetically pleasing outcome. 
Swelling at 4 months? Breathing diminished. Had one steroid injection with no results at 3 months.

It appears that you had rhinoplasty. I recommend following up with your surgeon however if you are not satisfied then I would suggest a second opinion. It is sometimes helpful to get a second opinion to see if there are other recommendations to try and achieve the optimal result.

If you have concerns then I would suggest that your seek out several second opinions from highly experienced rhinoplasty surgeons. It is never a problem to get additional information so that you can make an informed decision on how to proceed if your are not happy with your initial surgery. It would be helpful to obtain your prior medical records so an accurate assessment can be made.

Polly beak can be caused by multiple factors. The most significant is the surgeon not building in enough tip support during the surgical procedure and during the healing phase or over time the tip drops creating the deformity. Additional factors that predispose are thick skin, not having enough compression on the tip during the immediate post operative period and as a result allowing blood to collect under the tip of the nose. It is still very early in the recovery process and swelling is very common during the initial recovery. I would however ask your surgeon to assess for possible fluid or possible blood  in the soft tissues as this can significantly delay the healing process.

There was significant swelling on your 8 week post op photo so it suggests that there is significant post operative swelling that will take the full 12 months to resolve.
