A columella strut is used to give tip support and prevent the tip from falling so much during smiling. To obtain projection, a tip graft is placed on the domes or tip cartilage angle. Osteotomy’s are the most traumatic event while performing a rhinoplasty but when necessary to give the best results should be performed.

To answer your question, yes cartilage can be taken from the left side if the right side was used and it is determined that there is not sufficient cartilage remaining on the right side. But, I think that is highly unlikely. More importantly is can your skin on your nose withstand another surgical elevation without permanently changing or hurting the skin. Only your revision rhinoplasty doctor can help you determine that.

The structure between your nostrils is called your columella, Everyones nasal tip falls as we age because the columella cannot hold the tip up. It sounds like no tip support was added to your nose after your revision rhinoplasty. Sounds like a columella strut should have been placed. I inset one on almost every revision rhinoplasty I do.

The lower one third of the nose is made up of two cartilages which form the nasal tip. The structure between the two nostrils is called the columella. The tip is shaped like an arch. You said the bridge of your nose was lowered and the breathing is great. It is not uncommon to lose tip support when the nasal dorsum is unroofed during your primary rhinoplasty. Because the tip is shaped like an arch, as it sets back, it rounds out. Sounds like you need tip support re-established. Maybe a columella strut and/or tip graft. This will give you better tip definition.

Several thoughts. Only have anesthesia if your doctor says it’s required. Talk to the anesthesiologist also. You should be medically cleared by you family MD or internist. There is no basic inherent increased risk with anesthesia after rhinoplasty. Be sure your doctors know you had a recent rhinoplasty. Remind them at the time of surgery. Anesthesiologist like to put masks on patients faces and this could affect you recent nasal bone work.

This is a difficult situation because you have already had three procedures on your nose and you are not healed from your last procedure. You need to wait six more months to totally heal. Then you need an evaluation by a surgeon who regularly performs revision rhinoplasty. A correct diagnosis must be made. Then an accurate treatment plan established. Can what you want be achieved without risking your present result. Only an in person evaluation can establish that.

You have a mid nasal dorsal fullness after rhinoplasty. This is probably residual septal or upper lateral cartilage. The problem with steroid injections is that it will take down the soft tissue around the bump and base relief the fullness out. Make it more apparent. Injectable fillers above and below the bump may help or the cartilage bump may have to be resected at surgery.
