What looks good on the model, may not look good on you. A consultation with digital imaging will help you confirm your desires and an exam will tell you if it is reasonable.  The balance of a small chin implant and a reduction of your nasal dorsum would be a nice improvement of your profile. Thank you for your questions.  I don't know why you are still experiencing significant swelling 4 yrs after a rhinoplasty. If the swelling is intermittent, you should try to take some photos of your nose at these times as well as when it looks normal again.  I recommend you seek out a rhinoplasty specialist for a consultation.  You should present the pictures, so that you can share your concerns even if the swelling isn't present at the time of your appointment. If you are unhappy with the results, you should communicate them with your original surgeon if possible.  Reviewing before and after photos with him/her may are beneficial. If not, I would seek out a rhinoplasty specialist in your area and get a consultation. As for ear (auricular) cartilage, in specific situations, it can be used effectively. I am sorry you are still having issues with your nose. Without a clinic visit and an exam, it is difficult to know if something can still be done for you.  If your nasal is in good shape, even if some underlying scar tissue is still present, then there is a good chance that further options are still available.  I recommend seeking out further opinions from a rhinoplasty specialist in your area.
