The bottom line is “it all depends”Generally speaking if the patient’s decision is to separate the two procedures I prefer to recommend treating the aging face first (face lift) and then set the rhinoplasty goal to the post op facial proportion and result. I agree that this is generally unusual 5 weeks following surgery. There may be good reasons for this depending on what you had preoperatively and what was found and performed during surgery. I would make a follow up appointment with your surgeon and review your concerns. I would expect he/she is just as concerned as you are and interested in your happiness. Kamaira: I too would recommend that you approach this conservatively. My suggestion would be to first try Botox™ and an indictable filler like Juvederm™. Dorothy: Without a thorough examination, it appears that the optimal cosmetic result would be with an Endoscopic Brow/Forehead lift and a conservative upper lid skin excision combined with laser resurfacing on the upper and lower lid skin. Joy: good post! First of all you have beautiful green eyes and the photos are good.

Your issue is at the lid-cheek junction and tear trough area. Without seeing the whole mid face area (cheek bone) i would likely suggest 1. First laser the lower and upper lid skin going slightly on to the thicker cheek skin using a light to mild carbon dioxide laser to tighten the skin and then reappraising the response after 8 weeks. I would expect that you are in your 30’s.

Based on a photo and not an examination, I would suggest leaning towards fat transfer. I have used implants since 1984 and Fat Transfer since 1986, I have found fat transfer performed properly with periumbilical fat to have exceptional long range results in the tear troughs.
