One of the main reasons I’m not a big fan of non-surgical rhinoplasty, also the results are usually less than amazing when seen from more than one angle / in person. And you’re limited in terms of what you can do with reshaping compared to surgical rhinoplasty. So the risk / reward ratio is kinda off

Hi you really can’t make any judgements whatsoever with the cast on. Even once the cast comes off you still have a ton of healing to go through, so best to reserve judgement until more time passes.
I take photos immediate post op for my patients to be able to see their changes right away, which can help allliviate this type of anxiety. Perhaps your surgeon has something like this.

There can still be significant swelling at 6 weeks, even if it feels firm there is callous formation and bone remodeling ongoing. You should wait a full 12 even 18 months and then reassess the potential need for revision at that point.
I take immediate post op photos to show my patients the amount of hump removal as seen on the table. Perhaps your surgeon has something like this which may help answer your question for now.

Well to answer your question as to why, it’s basically a scramble of genetics and how your face grew. Essentially luck of the draw. No two noses are ever exactly alike.

Good news is it can be fixed. One of the most aesthetically important things I do during surgery is improve symmetry. Doing so will eliminate much of the differences you see on the angled views from each side.

You do ask a very interesting question. I could see how it would be confusing. The problem with over-deprojecting your nose, even with not-so-thick skin, is that the skin envelope might not shrink wrap over the smaller framework. In this case, if you deprojected too much then the excessive skin would just sit on top of the cartilage, and you would lack definition which gives the nose a pleasing appearance from the front as well as the 3/4 views. I would agree with your observation of an over resected bridge, wide upper 1/3 with a likely open roof defect. Although you are still very early after surgery, these particular issues are not going to improve. In fact they could only get worse. The answer to your question is yes, we do this all the time. Typically the parts of the procedure which are covered by insurance are internal nasal issues, such as a deviated septum or turbinate hypertrophy. This will allow the insurance to cover some of the necessary parts of the cost including the facility fee and part of the anesthesia fee. Septoplasty (internal work for breathing) alone is always covered by insurance. I’ve never seen it denied. Cosmetic changes externally are not covered by insurance. The benefit of doing them at the same time is that the insurance pays the facility fee and some of the anesthesia, which means the overall cost would be less as opposed to doing them separately.

I use spreader grafts of customized thickness maybe 50% of the time, typically for asymmetric / deviated noses. You are correct that blindly placed spreaders in all cases without concern for the mid-vault volume is definitely a mistake. Lack of attention to mid-vault however can cause post-operative internal valve collapse or inverted V deformity, especially with dorsal hump reduction.

Yes you can definitely narrow the tip and it would give you more balanced look. I would also consider narrowing the nasal bones too depending on how narrow you want the tip / the look you prefer. A deviated septum typically refers to the part of the septum internally. This would impact breathing and can be fixed with septoplasty.The external portion of the deviated septum can affect breathing but typically is more of a cosmetic issue. You could fix this at the same time if so desired, but its not necessary in most cases. Hope this helps!
