I would say you are a typical good-great candidate in my practice. You are young and do have less full cheeks. Assuming you have reasonable donor site fat (rear end, hips) then you are exactly the type of patient that tends to do well with fat transfer.Keep in mind that the “dark” circles, can be 2 different phenomena. I do a good number of fat transfers and actually the lower eye / orbital rim (upside down C area) is one of my favorite areas for fat transfer. I have had similar (albeit smaller) lumps over the years and have found them to almost always respond to very diluted injections of triamcinolone. Low concentrations of the steroid are important and waiting about 6-8 weeks between injections has worked well for me in these related situations. Best of luck to you.

I have injected 100s of patients with Sculptra but have never found Sculptra to last “forever”. I have seen patients have it last 2-3 years; but usually even when it is lasting a long time, they want to come in and have a “booster” vial(s) around 18-24 months. There is not a true “Sculptra Reversal” injection (like Wydase to Restylane). I would be cautious with off-label injections. Perhaps consider VERY carefully performed lower face liposuction, or a “lifting” surgical procedure. 3-4…

In general I would say “not exactly”. For one, if you have fat in your “lower” face I would be very cautious about using liposuction to remove that. In my experience, liposuction is great for the neck but should be used with caution in the face per se. Also, if you want fat in the “upper” face; I would prefer that fat be grafted and come from a donor site such as the abdomen, flanks, or buttock area. Short answer to your question is I think that is an excellent spot for Sculptra. It’s off label in that particular area so make sure you consult with someone who does Sculptra a good deal. Sculptra is very different from other “gel” fillers or Radiesse. You want someone who has done hundreds of injections. Perhaps a Perlane or Voluma would also work in that area. Your one picture does not give me a great “3D” sense of the entire area of cosmetic concern. But again, short answer is Yes……

I am not a legal expert on this but this is my current understanding. I have heard of surgeons that do “save” some fat (frozen typically) for later injection. However, technically (again, my understanding), is that if a doctor does this they are acting as a “tissue bank”. Its impossible to be exact by just looking at a picture. That being said, you may just have “normal swelling” after Sculptra- which can take up to 2 weeks to go completely away. There is another less common condition often called malar lymphatic edema (MLE). It can also look similar to that.
