From the photos, it appears that you are a good candidate for revision rhinoplasty. I feel that most if not all of your concerns can be addressed and improved. The length and droop can certainly be improved. The crookedness can usually be improved as well, although it looked like there was some inherent crookedness to begin with. Using fillers to hide a bump is certainly an option – the effect usually lasts 6-8 months and people like it because there is no recovery. Obviously, a rhinoplasty will give you a permanent result, but it is a lot more expensive and 1-2 weeks of recovery is needed. You should consult with a rhinoplasty expert to get the best opinion.

The answer is that it can help, but is usually not the only thing that needs to be done. If the nasal tip is droopy because the lateral cartilage is really long, then the lateral cartilage must be shortened or the tip won’t come up. Just because you have thick skin does not mean you can’t get a nice improvement with rhinoplasty. It’s true that someone with thinner skin may get a more dramatic result, but a well performed rhinoplasty should certainly make the tip look thinner. While no surgeon can guarantee the second photo as your outcome, it is certainly a reasonable goal to try for. Your bridge will need augmentation. We precisely shape a Gortex implant to do this. In the hands of an experienced rhinoplasty surgeon, a revision rhinoplasty wouldn’t be considered “risky.” It’s really a question of how good of a result are you going to get. I would imagine that your second surgeon had to rebuild some of the cartilage that was removed from your procedure in the 90’s. I don’t see any photos for this question, but it general, the tip has to be as well as the nostrils for a wide tip. In general, you want the nose to have a triangular shape, and if you only reduce the nostrils, the nose can actually look more square.
