Face lifting is as much an emotional as a physical procedure . If you feel your appearance is holding you back from living to your fullest you may be a candidate for a procedure now . I have seen very young women 40 be very happy with a less invasive lift . The key is to match the right procedure with the right patient .
A large bone near my nose is hard and hurts. I had rhinoplasty 2.5 weeks ago.

It is common to have pain and irregularities after surgery on the nose , I would recommend bringing this up to your surgeon , occasionally a massage of the region can help if you feel a “bone” ... I wouldn’t be concerned unless this persists after 6 months

I appreciate your concern following surgery ,I do not feel your nose will get longer or go down with time . It often takes a while to get used to a new look but my patients usually can tell if “it’s right “ , even with a cast in place .. I would allow a minimum of 6 months for this to heal and if you’re not happy consider a revision

A deviated septum may cause a whistle in the nose . If a deviation is caused by a traumatic event the external nose is often deviated as well .Insurance will usually cover deviated septal surgery but often make getting coverage for external nasal repair difficult , classifying this as cosmetic. I would recommend going to a double board certified ENT/Facial Plastic Surgeon for another opinion .

The use of simulated images may help you focus your thoughts about your surgical expectations ,however exact replication of a simulated image should not be expected . Post surgical outcome will usually approximate a professionally simulated image and I would encourage you to see a Board Certified Surgeon and have one done.

You have a good chance of getting back to work in 2 weeks , many things influence healing time such as having an open vs closed procedure ...returning to work also is dependent on how you feel after surgery , will you be mentally ready to show off your new nose?

You will notice a change after a rhinoplasty .. prior responses have suggested correcting droopy eyelids (Ptosis) and possiblely augmenting your chin , both may improve facial balance and harmony and would be worth exploring during a consult

You do have signs of thick skin . Most surgeons suggest 1 year as a waiting period for final results . I would recommend you follow closely with your surgeon and be as open and candid about your concerns.

The midface is most important

A tired appearance is usually related to the midface dropping , and more specifically the creation of deep shadows in the tear trough area . Anything you do to improve the infra orbital shadowing will make you look more "refreshed". Injection of filler , fat , a midface lift or laser will help . That is probably why you are getting many different opinions . I would recommend the most minimally invasive treatment , unless you are eager to have a permanent result then opt for surgery

Your expectations are reasonable and should be achieved with a modern rhinoplasty .My recommendation is to decrease your dorsal hump so that it does not distract people from looking directly into your eyes, other details should be discussed at the time of consultation, including nostril size and rhinion augmentation .
