Breast Implants and Lift to achieve tear drop shape?
Avoid the Doughnut Breast Lift
Mar 1, 2010
I am sorry to pick on you, but I think that we need to be honest with each other.

You may be overweight. It's difficult to "size" breast implants in bigger gals, especially if they are young and their weight might continue to fluctuate.
Your breasts already appear pretty big. Do you really want implants to make them even bigger?
Your asymmetry is significant. The breasts are not only different sizes, but they are shaped differently.
Periareolar breast lifts (also known as...

Refining nose sides possible with a nose job?
Rhinoplasties are best planned with additional photographic views
Feb 14, 2010
Saphira,
Sounds like and looks like that you would benefit from a surgical rhinoplasty. You are very attractive, and a nose job would help to harmonize your nose with the rest of your face.
However, I can't give you specifics without side views ("laterals") and a "worm's eye" view (from your feet looking up into your nostrils).
I suspect that you will require a full rhinoplasty, involving the tip, sides, and top....

Usually, I make a small incision in the skin of the colum

Best procedure for excess fat and skin after Lapband surgery?
A Full Tummy Tuck will best correct the upper belly and belly button
Feb 13, 2010
First and foremost, congratulations on your weight loss! What a wonderful transformation. In addition to your improved health, your figure already looks terrific.
Second, even though you might think that your tummy has "unique" issues, nothing could be further from reality. I see at least one patient per week whose tummy looks like yours.
Third, go for a full tummy tuck.

After 4 years, your skin is not going to shrink on its own any farther--no matter how much exercise you do...

Possible to have a breast lift while awake?
Breast Lifting without general anesthesia possible, but not recommended
Dec 19, 2009
Yes, it is possible to undergo breast lifting without general anesthesia, but I would not recommend it.
Many of my colleagues inject tumescent fluid (very, very dilute "novocaine," similar to what is used for liposuction) into breasts for augmentations, reductions, and lifts. However, I have some concerns with this technique:

The surgery remains painful. It is very difficult to make the breast totally numb.
Tugging and pulling are a normal part of surgery. Will these...

Tummy Tuck for skin, fat, and stretch marks after pregnancy?
Weight loss and Tummy Tuck will give terrific results
Dec 19, 2009
At the time of surgery, patients should be at their ideal weight. Sometimes, this is signficantly more than their primary care doctors tells them. I try to be realistic with my patients. While they might be most healthy at 135 pounds, that weight may not achievable. So, we accept 186 pounds.
I suspect that, at the time, Peachy, that you return to 186 pounds, your stretch marks will actually be worse. Sorry to be a jerk, but the fat in your tummy is "inflating" the skin, so that it...
Is Breast Lift by skin excision possible?
Most Breast Lifts maintain current sizes
Dec 19, 2009
Yes, there are a variety of techniques for elevating the nipple-areolar complex without reducting the breast size significantly. I favor a "lollipop" incision, which sometimes requires a short horizontal extension (to create an up-side-down T).
Breast lifts usually do require the removal of some skin and a little fat. I doubt that these small amounts would radically change your breast size.
However, most breast lifts are most attractive with implants. Lifts really only lift the...
Will smoking marijuana affect Tummy Tuck surgery?
Smoking Marijuana decreases the oxygen supply to your healing tissues
Dec 14, 2009
Any type of smoking will diminish the oxygen supply to your healing tissues. Smoking is especially important to avoid before any surgery that involves the creation of flaps (complex segments of tissue with their own blood supplies), such as face lifts and tummy tucks.
If you were about to undergo upper eyelid blepharoplasties, routine breast augmentation, or liposuction, you would probably get away with smoking and yet still have a good result. However, I would probably cancel your tummy...
Transsexual breast augmentation
3 Tips for Transsexual Breast Augmentation Surgery
Dec 12, 2009
Show up with letters from 2 mental health professionals, who support your decision to undergo sexual reassignment surgery.
Bring in a letter from your endocrinologist or internist. The note should document your compliance with your hormone regimen and should confirm that you are healthy enough for a general anesthetic.
Plan on getting silicone implants in subglandular positions (not beneath the muscle).

Good luck!

Is it better to have Tummy Tuck done locally?
Stay close to your surgeon
Dec 12, 2009
If you are going to have surgery out of town, then plan to stay there for a couple of weeks. Your doctor won't be able to treat your complication and to answer your questions if you are hundreds of miles away.
About 5-10 times per year, I operate on patients from out of state. My only requirement is that they stick around for the first couple of weeks. While complications are unusual, they can be so easy to treat if the patient can just pop into my office. On the other hand, from a distance...
Laser tummy tuck after weight loss
Lasers are often more hype than good medicine
Dec 12, 2009
A laser tummy tuck? What the heck is that?
I am a Board Certified Plastic Surgeon, and I have been in practice for 8 years, and I have no idea what a laser tummy tuck is. Whenever I read about anything having to do with a laser, I am usually skeptical; 9 times out of 10, the laser practitioner is not a Board Certified Plastic Surgeon, and he is relying on a gimmick to get your business.
Sorry to pick on you.
Now for some substantive advice: Given that you have lost a lot of weight, and...
Lower vs higher cc for high profile silicone breast implants?
Difference between 350 cc and 375 cc is minimal
Dec 12, 2009
I think that you have done your homework! You have spent a lot of time providing a narrow range of choices for your plastic surgeon. Now, step back and trust him. The difference between 350 and 375 cc is just 7%. Consider that you may have another 200 cc of natural tissue, so you are really debating between 550 and 575 cc, which is just a 4% difference.
At this point, I would recommend finding some photos of "ideal" breasts and handing them to your surgeon. Your doctor can use...
Lifting kids after Tummy Tuck?
After Tummy Tuck, no lifting more than 10 lbs for 6 weeks
Dec 12, 2009
If you undergo a tummy tuck, wait 6 weeks before you lift anything greater than 10 pounds. You do not want to stress the repair of your rectus diastasis (the stretched portion of your "six-pack" abdominal muscles).
I would also recommend that you wait until your son is at least 6-12 months old. It's remarkable how much natural skin shrinkage women obtain after giving birth. Breast feeding really helps get rid of the extra weight, and is good for your baby too.
Why are Tummy Tuck incisions so high?
Tummy Tuck Incisions Can Be High or Low
Dec 3, 2009
The incision for a Tummy tuck can be placed low or high, depending upon the patient's goals and her physique.

I always incorporate C-section scars, appendectomy scars, etc., into the tummy tuck.
I try to keep the scar at least 7 centimeters above the fourchette (i.e., clitorus) to avoid distortion of the pubic mound.
Usually, the incision is best angled towards the anterior superior iliac spines, but can run slightly lower. The lateral (outer) portions of the incision must be above the...

Cat's eye look surgery?
Canthoplasties Can Produce a More Feline Shape to Eyelids
Dec 3, 2009
When performing lower eyelid surgery, I usually tighten the lateral canthus (the outer junction of the upper and lower eyelids) to prevent ectropion (the pulling of the lower eyelid away from the eyeball itself).
If a patient has an established ectropion (from nerve injury, scarring, age, or whatever), I might tighten the lower eyelid even more with a procedure called a lateral tarsal strip canthoplasty. However, I have never performed this surgery to change someone's "look." ...
Would Restylane on nasolabial fold affect Rhinoplasty?
Restylane and Rhinoplasty are safe to combine
Dec 3, 2009
I can't see why these 2 procedures couldn't be combined. Even with an alar base reduction, the aesthetics of the nasolabial fold are (mostly) distinct from the nose.
However, if you were my patient, I would probably separate the Rhinoplasty and the Restylane. The swelling from one might affect the fine performance of the other. The injection of Restylane, in particular, is so routine in my office, that I would feel very comfortable completing your rhinoplasty, and then doing the...
