Answer Snippet
Will nose come down?
Upturned nasal tip
Jul 24, 2015
It is extremely common that patients will feel that their tip is upturned too much after rhinoplasty. Indeed, we try to tape the tip in an upturned nature to support the tissue healing. We always see the tip come down to the level that was developed during surgery within 6 months, perhaps longer, as the nasal tissues relax during the healing process. We also ask our patients, in a good natured way, to avoid starring at their new nose for more than 2 hours daily, otherwise one can over think…
Nose collapsed after septum harvested. What do I do?
Nasal collaspe post op.
May 13, 2015
Your description implies a complex nasal injury has occurred and it’s difficult to express your needs technically without an examination. Both ear cartilage and rib cartilage can be used as needed. You need to seek out an experienced nasal surgeon to evaluate your nose and provide you with all your options.
Do I have a deviated septum?
Crooked tip and septal deviations
May 10, 2015
Your submitted picture does show a deviation of the tip. Causes could include a septal deviation and/or asymmetrical tip cartilages. Both conditions can be “diagnosed” and corrected at the time of an open rhinoplasty, as is commonly indicated. While each nose is different, an experienced nasal surgeon should provide you with excellent results.Hope this helps.
Is ThermiTight an option to treat malar bags?
ThermiTight for malar bags.
May 10, 2015
This is a great question without a definitive answer since it is still too early for the use of this treatment for this condition to evaluate results, risks, and longevity. Theoretically, there should be some benefit, but proper protocols regarding temperature settings and recovery have not been established. Other alternatives include fractionated CO2 laser peel with volume replacement of the cheeks. Always look at before/after photos and talk to patients personally before deciding on a…
Is ThermiTight effective and permanent?
Longevity of ThermiTight
May 10, 2015
We feel that our results are lasting over 2 years. As our experience grows, this may prove to last longer. But since we continue to age, nothing lasts forever, and the results following any revision would be expected to be just as satisfying. Although technique dependent for optimal results, ThermiTight is an excellent alternative for selected patients who want skin tightening with minimal risks and recovery.
Open or closed surgery? What changes could be made and how? Is it possible to make my dorsum wid
Open approach preferred
Dec 24, 2014
The open approach leads to better exposure of tissue, and ability to perform structural rhinoplasty techniques which produce more stable and longer lasting changes in nasal appearance. This permits utilization of any further techniques that become needed during surgery, such as spreader grafts or tip grafts to correct lack of tip support. Get as many consultations as you need to feel comfortable with your surgeon. I might also suggest find a doctor that can show you computer imaged…
What would your recommend for dorsal hump removal?
Dorsal hump and tip work
Dec 11, 2014
From your one profile picture, it appears you would benefit from slight nasal tip uplifting, which also produces an apparent decrease in nasal size, along with a slight reduction of the nasal bridge. An office with a computer imager may be helpful to communicate with your surgeon exactly what you desire. I stress to my patients that the post op appearance should be esthetically pleasing and look natural, with the end result that nobody should actually be able to tell that you had nasal…
I want my nose fixed, but need to know a little more about cost and the type of procedure I need
Costs can vary
Sep 3, 2014
Your only picture submitted demonstrates on frontal view a deviation of the bridge. This can be caused by previous trauma resulting in a deviated septum or or a displacement fracture of the nasal bones. I cannot tell if your tip is drooping or needs work by this one picture, but can be determined and explained with a proper examination. The surgery, using up to date techniques, can be done on an outpatient basis, with almost no discomfort, and with very minimal swelling and bruising. Patient…
I hate my side profile. I’ve always have had low self esteem issues cause of this.
A typical facial inbalence situation
Aug 19, 2014
Most facial inbalence issues arise from a weak chin, which inversely provides a perceived visual enlargement of the nose. If the face is divided horizontally into thirds, the upper third (the forehead) usually is not a problem. The middle third (the nose) and the lower third (the chin) are the common areas treated. Options include an anatomic shaped chin implant, or a sliding genioplasty (a bone cut surgery that slides the lower jaw forward). This is followed by a nasal reduction of a least…
ThermiTight: Amount of treatments required?
ThermiTight multiple treatments
Aug 18, 2014
Every aging skin differs in the ability to contract from a thermal effect, and patients differ in the amount of skin needing tightening. I tell patients to wait at least 3 months, perhaps 6 months, and if not satisfied, the treatment can be repeated safely, and at a discounted price, which results in even better improvement. As with any non-surgical treatment, results may be less dramatic than surgery, but without the potential prolonged healing and scaring that goes with surgery. I…
I want to get my hanging columella fixed. What would you recommend?
Correcting a hanging columella
Aug 17, 2014
A review of your pictures indicate a mild hanging columella and a wide nasal tip. A week chin also accentuates the nasal appearance. The columella can be shortened or lifted with a mild shaving of the end of the septal cartilage (the caudal edge), a mild resection of the membranous septum (the skin between the septum and the columella), trimming the feet of the medial crus (cartilage that sticks out into the nostril) or thinning the width of the medial crus cartilage. This surgery can easily…
Does a deviated septum get better or worse over time?
Dislocated septal tissue usually needs surgical correction
Jul 6, 2014
Any trauma to the nose could cause a dislocation of the bendable cartilage of the septum, which in turn would cause blockage of the nasal airway. Unfortunately, treatment is a surgical septoplasty, since the septum is technically not broken, but displaced, and will not straighten itself . Hope this explanation helps.
Will adding height to the dorsum add more definition?
No simple answers
Jul 5, 2014
As with most post operative evaluations, there can be multiple solutions to your concerns. Obviously, you have provided limited pictures and definitive answers would entail an in-person evaluation at your surgeon’s office. I agree that your dorsum looks flat and wide. Lateral osteotomies and mild onlay grafting to the bridge will make the nose appear less wide and provide definition. Your tip looks slightly elongated or droopy and also lacks definition. Both columellar and tip grafts can…
Question regarding importance of Revisions preformed by Board Certified MDs.
Board Certification does not equate with competency
Jun 29, 2014
Your question underlines a major misunderstanding by the public. Mainly, competency to perform a certain procedure is not tested or examined by any board certification process. Indeed, many board certified plastic surgeons have had little or minimal training and experience in rhinoplasty (and perhaps other cosmetic procedures also). The Am. Board of Facial Plastic and Reconstructive Surgery (ABFPRS) and the Am. Board of Cosmetic Surgery (ABCS) perhaps demand the most knowledge…
Has any Dr heard of temporary Empty nose syndrome after turbinectomy and tip plasty?
Probaly results are permanent.
May 24, 2014
Your condition, also known as “ozema”, is caused by the inability of the nose to clean itself. The turbinates have microscopic hairs (cilia) on the surface which beat in waves and carry mucous secretions back to the throat where they are swallowed unconsciously. If too little turbinate tissue survived the surgical trauma, dryness and whistling inside the nose may develop. I do not feel that swollen turbinates would affect the tip results. Your ENT doctor should be well aware if the…
