I do not see your columella as wide from the inferior view, which is good because that is a largely uncorrectable complication of open rhinoplasty that cannot be repaired. However, it is low as your nostril contours are irregular. Rib is the most difficult building material to work with for the nose, but the fortunate thing is that as we age, the rib becomes more calcified. That is actually very good because it makes the rib much more stable. At your age, your rib should be excellent. The important thing to remember is that the inverted V is not a cosmetic problem: it is a manifestation of collapse of the internal valves, which almost always happens when the height of the bridge is reduced. I don’t completely agree with your observation. Your radix height was originally good – – it was at the level of your upper lash margin. The reason you don’t like your current appearance is that you had a very shallow, almost nonexistent, depression between your nasal radix in your forehead (frontonasal depression). You have very smartly made the right diagnosis. When the bridge is reduced, that resection takes off the cartilaginous roof, which means that the sides of the nose in the middle third (the internal valves) no longer have the support that they did, and will fall toward the midline, obstructing the airway and making the middle of the nose seem narrow compared to the bony part.

I see very different features than you do. Your eyes are strikingly pretty, and the small shadows underneath are not hollow areas, but simply normal anatomy– the place where the muscle joins the edge of the bone.. One of the fallacies upon which rhinoplasty has always been built is that if the surgeon makes a perfect skeleton, the skin will take on that shape. Unfortunately, it’s not that simple.
