I don't think its unusual for someone in your situation to be apprehensive about surgery. As long as you are having this operation in a safe environment, with a qualified (board certified) plastic surgeon, you should be ok. I am not sure if the hollowing of your cheeks is due to smoking. But whether it is related or not, there are many options for filling the cheeks out. You can use temporary fillers or more long lasting fillers. Your options with temporary fillers (lasting anywhere from 6 months-2 years) are Juvederm, Restylane, Radiesse, Voluma, or Sculptra. Fat grafting is longer lasting. You can depend on at least 2-5 years, and sometimes the fat can last longer than that. But as of now fat is the best...

Based on your picture, I doubt that you will get the look you are looking for. Your breasts are sitting low on your chest. Large implants will fill you up, but you upper chest is not going to fill up. You almost need a breast lift for getting that "fake breast" look that you are talking about. The size of implant will depend on several factors including your baseline breast tissue, you height, you chest width/lenght, quality of skin ect. It seems to me that you want to know the difference between doing an operation (Tummy tuck) with an umbilical floating skin excsion vs. no skin removal at all (only repairing a loose muscle). If so, I would say your recovery is essentially going to be the same. Because in order to do the muscle repair you still need a incision made in the skin. The nose shouldn't heal at different rates. If the nose feels different on one side, it might be because there is either cartilage graft or scar tissue underneath. At 7 months if it feels and looks noticeably different you should return to the surgeon to reexamine your nose.
