A standard septoplasty on its own wouldn’t be expected to make your external nose straighter, so if that’s a concern you’d likely need a rhinoplasty.

Without examining your nose it’s hard for me to say what would need to be done to improve your congestion symptoms. A large perforation can contribute to feeling congestion, but there may be other issues going on that would better address your symptoms.

You’ve waited long enough at this point that I wouldn’t expect further waiting to change…

Thanks for your question regarding your crooked nose. I can certainly see how your nose tilts quite a bit to your right. This is something that can be improved with a septorhinoplasty surgery. It’s very hard to get it perfectly straight but it should be possible to improve the positioning overall.

The question of whether the surgery is worth it is really a decision for you to make. Your crookedness is quite significant so if this bothers you I suspect that straightening your nose would be…

Yes, I can see from your photos how your columella is deviated to your left side.
This is commonly due to a deviation of the septum in that area.
Septorhinoplasty surgery can be used to correct the septum position and improve the columella symmetry.

You should have a preoperative clearance with either your primary care doctor or a cardiologist to make sure the EKG finding is ok. Your rhinoplasty surgeon wouldn’t typically be able to make that assessment.

Yes, it is possible to address your asymmetrically elevated nostrils. It would involve a revision rhinoplasty as there isn’t a small quick fix to adjust the nostrils unfortunately.

Based only on your photos your nasal tip skin doesn’t appear severely thick, but it would be best to examine the area in person to verify this. It does seem as though a large portion of the roundedness to your nasal tip is from the curvature of the nasal tip cartilages.
