Are spreader grafts necessary in nearly all cases when dorsal hump reduction is performed?

Spreader grafts can be used to help maintain the nasal airway stability in the middle third of the nose.  When a dorsal hump is removed, it can weaken this portion of the airway and pose a risk for breathing obstruction in the long term. Spreader grafts are often used to help prevent this complication from developing.  Smaller dorsal humps can often be removed without weakening this support and may not need spreader grafts.

It sounds like this is an inflammatory reaction to the rubbing from the goggles. I would try to switch to a goggle with a different nose piece and does c=not come in contact with that area.  You can continue to massage it as well and if it doesn’t resolve in a few weeks consider an in-office consultation.
How long will it take for a nasal osteotomy to heal?

It is still too early to tell whether or not there will be any permanent sequelae to your recent injury.  The swelling from your surgery appears to be normal at this stage and although it is hard to be patient, I think time will tell.  Make sure to follow-up with your surgeon and let them know of the injury.

A tip rhinoplasty focuses primarily on modifying the tip of the nose including possible resculpting, reshaping, straightening, refining. It can be difficult to tell if you have sustained a nasal fracture after facial trauma.  Often the nose may look bruised and swollen but may not truly be broken. Sometimes the nose will look shifted and you may feel the bony fracture lines which suggest a fracture.  However, these changes may not be noticeable until swelling starts to subside. A consultation with your doctor will help determine if the nose is fractured and if so, what needs to be done and in what timeframe.
