After a septoplasty, please advise me if I can use a cotton bud to remove the crusting by myself. 1
Care After Septoplasty
Mar 18, 2018
Thank you for your question.  I recommend reaching out to your surgeon to confirm, but in general, it is helpful to gently use cotton-tipped applicators dipped in hydrogen peroxide to remove crusting in the nostrils after septoplasty.  Keeping the nose moist with nasal saline spray as well as Ayr saline gel or Aquaphor is also very helpful during the healing period.  Hope that helps, good luck!
Should a doctor know where the arteries in the face are to avoid causing blindness or skin necrosis
Avoiding Filler Complications
Mar 18, 2018
Thank you for your great question.  While the risk of arterial injection is low, the risk of complications from arterial injection are exceedingly rare in experienced hands.  In other words, inadvertent arterial injections can occur, but when recognized and treated immediately and appropriately, long-term side effects can be avoided.  Ways to minimize risks include microcannula approach and choosing an expert injector who injects slowly and carefully and aspirates prior to injection....
Are the results after one week of septorhinoplasty close to what the final results may be?
Swelling After Rhinoplasty
Mar 18, 2018
Thank you for your question and photos.  It is normal for patients to feel that their nose looks "really fat" one week after a septorhinoplasty!  Swelling will decrease substantially over the next 4-6 weeks-- so you have a lot to look forward to.  By three months, approximately 90% of the swelling will be improved, but it will take at least 12-15 months for your "final shape" to take place.  Hope that helps, best of luck to you!
Does having lip filler help with those ‘smokers’ wrinkles on the top lip?
Lip Filler for “Smoker’s lines”
Jan 20, 2018
Thank you for your question and photo. Lip fillers such as Juvederm Volbella and Restylane Silk are a great, longer-lasting treatment for smoker’s or lipstick lines/wrinkles around the mouth. You could also consider layering additional filler to build  volume in your lips as well. Hope that helps!
Question about platysma muscle in my neck: would a vertical facelift or deep plane facelift be a be
Revision Facelift
Jan 5, 2018
Thank you for your question and I am sorry to hear that you are disappointed with your results.  The optimal approach for your revision face/necklift would be best determined by an in-person consultation to evaluate your anatomy/current features as well as goals of care.  In general, the vector for lifting the face is ideally more vertical than horizontal (regardless of deep plane or not), although this can vary somewhat based on age and anatomy.  I recommend reaching out to your surge...
I can’t afford rhinoplasty right now and according to my ENT I have to have septoplasty ASAP. Is it
Timing of Septoplasty with Rhinoplasty Interest
Jan 4, 2018
Thank you for your question and photos.  If you are definitely considering a rhinoplasty in the future, it would be beneficial for you to hold off on the septoplasty if possible, because septal cartilage is often utilized during rhinoplasty.  Depending on your symptoms and exam, you could consider medical treatment (i.e. for allergic rhinitis, which would not resolve with surgery anyway), inferior turbinate reduction, and/or sinus surgery in the interim.  Hope that helps, best of luck to you!
Why do I have chronic nasal congestion after septoplasty and turbinate reduction surgery?
Persistent Nasal Congestion After Nasal Airway Surgery
Oct 31, 2017
Thank you for your question and I am sorry to hear that you are experiencing these issues.  Persistent nasal congestion following septoplasty/inferior turbinate reduction can result from: nasal valve stenosis and/or collapse that was not addressed, underlying chronic allergic rhinitis (not adequately treated medically), underlying chronic sinusitis, re-deviation of the nasal septum, re-enlargement of the inferior turbinates, etc.  In terms of the external appearance, it may be related...
Is it a deviated septum if I can breathe out of only one nostril at a time?
Nasal Obstruction
Oct 31, 2017
Thank you for your question.  Alternating nasal congestion is a normal entity (called the nasal cycle) but it is not necessarily normal to be able to perceive it.  Potential causes that may be playing a role include inferior turbinate enlargement, deviated nasal septum, nasal valve collapse, chronic allergic rhinitis, etc, and it is certainly possible that you have more than one of the above issues.  You would benefit from an in-person evaluation by an otolaryngologist (ENT) or a facial pl...
Bell's Palsy: What would be involved to make the face not appear droopy and the eye to close?
Bell's Palsy
Oct 17, 2017
Thank you for your question. Fortunately, the great majority of patients with Bell's Palsy recover completely or near completely with time! If you are experiencing slow or no improvement, it is important to be sure that you do not have facial paralysis of another etiology (Bell's palsy is a diagnosis of exclusion, after all other things like tumors, trauma, etc., have been ruled out). If your eye does not close completely, it is important to protect the eye with...
Blowing and picking my nose after septoplasty, could I ruin my results?
Concerns After Septoplasty
Oct 17, 2017
Thank you for your question. Most likely, you did not "ruin" your septoplasty but I recommend reaching out to your surgeon to discuss your post-operative concerns. In the meantime, it is very helpful for patients with allergies to treat them with oral anti-histamines and nasal saline irrigations to cleanse the nasal airway, instead of blowing/picking. Hope that helps, best of luck to you!
I had platysmaplasty 05/30/17 and now have huge indentation under my chin. What went wrong and how
Cobra Neck Deformity After Necklift
Oct 17, 2017
Thank you for your question and photos. Some potential reasons for such an appearance after necklift ("cobra neck deformity") include excessive fat removal/liposuction centrally and inadequate skin elevation/draping. I recommend reaching out to your surgeon with your concerns as he/she may offer you a revision procedure in the future or provide you suggestions for a second opinion consult. An in-person examination would best determine next steps, but this...
How is it the scars from a mini lift or even a lip lift become so invisible?
Scars After Facial Surgery
Oct 17, 2017
Thank you for your question. Scars do not disappear completely but with meticulous closure technique and judicious post-operative care (moisturization, sun protection, silicone therapy, etc.), they can be very difficult to see! Also scars for mini lifts and lip lifts are well-concealed in natural transition zones in the face (i.e. preauricular/post-tragal crease and nasal base, respectively), which make them much more difficult to appreciate. Hope that helps!
Should the back of my ears look like this after a neck lift?
Incision Concerns After Neck Lift
Oct 11, 2017
Thank you for your question and photos. I recommend reaching out to your surgeon as I am sure he/she would be happy to hear from you regarding your concerns. It appears that the appearance behind the ears is related to the way the skin was draped, which may settle somewhat over time. However, an in-person examination of the left side may be warranted to ensure that there isn't fluid trapped underneath the skin causing that appearance....
Is it normal to have a yellow scab that turning greenish over the surgical inscion?
Incision Concerns
Oct 11, 2017
Thank you for your question. Probably not - I recommend reaching out to your surgeon to discuss your concerns and at the very least, text/email a photo of the area for evaluation. Hope that helps, wishing you the best of luck.
Can the tip of the nose be permanently large? Had a septorhinoplasty to fix a breathing problem.
Nasal Tip Swelling After Septorhinoplasty
Oct 5, 2017
Thank you for your question. I can understand your concerns regarding your post-operative swelling. The degree of swelling expected in the nose/nasal tip after surgery depends on what maneuvers were required during your septorhinoplasty in order to "straighten it out." Even in an endonasal or internal approach, some swelling is expected if grafts were placed or extensive work was performed in order to address a crooked nose. In general, it takes a...
