Laser tattoo removal, when performed correctly, is very safe and has infrequent complications such as the scarring which you describe. The R-20 “method” for treatment of tattoos is based on a very limited study, 12 patients, using only a single wavelength laser (755nm) which compared standard treatment protocols (1 treatment every 6-8 weeks) vs R-20 protocol (3-4 treatments in a single visit). I do not currently perform this treatment or recommend it to my patients as the study has significant limitations and has not necessarily proven itself as an effective way of decreasing the timeframe for adequate tattoo removal….

Your tattoo may be treated. The best laser for treatment is going to be one which has suitable wavelengths for treating red and yellow ink as well as the black outlines. In my office this tattoo would be treated with the Trinity laser system which provides three distinct wavelengths for optimum versatility and outcome. The choice between removing the entire tattoo vs the diamond would need to be discussed with you provider so that you could have an accurate assessment of timeframe and cost associated with removal.Your tattoo would do very well with treatment using the Trinity laser system (532, 694, 1064 wavelengths) due to the versatility of this system when treating tattoos with multiple ink colors such as yours. The best tattoo removal laser for you is the laser which will give you the most effective clearance in the least amount of time with low risk of complications and for a reasonable cost. The R-20 method is a technique which was published in a small study (12 pts ) where the patients tattoos were treated multiple times (up to 4 times) in a single visit waiting approximately 30 minutes between treatments.
