You do have a deviated nose and septum, but your main problem is something called the Tension Nose Deformity. This requires a rhinoplasty procedure to fix, and simple septoplasty will be completely inadequate for you. You need to have rhinoplasty to address this over-sized septal cartilage. Hi Anne-
In these pictures you have a classic tension nose, as seen at the upper lip and bridge fullness. The best thing to do is reduce the septal fullness at the bridge, and shorten the nose at the lip. Tip defining can be performed as well as bony improvements.

The Botox will fade over three months. Botox to the lower crows feet always weakens support of the cheek. I would avoid having this done in the future, but upper lid is fine and should be used for brow lifting.

I believe that your tip is droopy due to an overly long septum. The conventional answer is to add grafts but this may only partially help at best. Shortening and reducing the length of the septum and lower part of the bridge will help much more and minimally invasively. Unfortunately you will not find many surgeons who do exclusively this.

HiI am certain this is fixable without collapse if you have not used cocaine in your nose and not had prior surgery. Deviated septum at the base are difficult to fix for doctors without a lot of experience doing that sort of thing, but otherwise it is very do-able.ThanksHoward Stupak

You do have a deviated and raised bridge. These can frequently happen together because the septum exerted pressure upon these bones during growth. You probably have some breathing trouble too. This can be treated by repositioning the bones and septum from inside the nose.

Hi – You are a good candidate for a repositioning rhinoplasty. Your nasal septum is overgrown causing the deviation, curvature and pulls a little on the lip. The best way to deal with this is from inside the nose to reduce the size of the septum and bridge and reposition the septum and nose to the midline. Grafts and external incisions are unnecessary.

It is possible to narrow the wide bridge alone but it is important to at least assess the tip cartilages at the time of consultation as to whether they might appear widened once the bridge is reduced. Pyrosis can be repaired at the same time and sometimes requires consultation with an oculoplastics specialist.

Because of your functional problems, an inside the nose rhinoplasty will serve you best. The septum is bulging to the surface at the point where it meets the bone on your bridge possibly from growth or prior trauma. You probably have a septal deviation as well. Repairing the bridge and septum from the inside in minimally invasive fashion will serve you best.

Your problems are all due to septal overgrowth causing the deviation, the tip problems, the upper lip shortening and stretching and the hump on the lower bridge. An approach inside the nose without grafts to reduce the overall size of the septum will have the most natural and aesthetic results, as well as functional improvements.

You have a very specific nasal problem called a tension nose where the septum has overgrown creating deviation and pulling of the upper lip. Conventional rhinoplasty techniques do not work well for this, but resizing the septum and nose are very effective and can give complete resolution of aesthetic and breathing problems including snoring and sleep related breathing disorders. Start with the nose and then you can think of body surgery as you age in the future.

It is very possible to do an inside the nose rhinoplasty consisting of septoplasty, nasal bridge reduction and osteotomies. Open rhinoplasty is not required, nor is operation on the tip. Most important in choosing a rhinoplasty surgeon is not which approach is used, but the results, reputation, and experience of the surgeon specifically in rhinoplasty.

You have what is called a “tension nose” where the nose is pulling the tissue of the face forward and making it appear you have a small chin. Reducing the size of the nose and setting it back overall will help improve the lip size and stretching. You are a good candidate for this procedure.
