No mastopexy needed. As for timing, do you think the augmentation will have any effect on how you're judged when competing? The more time you give yourself to heal and get back into shape the better. Based upon photos alone a revision breast augmentation would be appropriate. Close the pocket out by the 'arm pit' and possibly open the pocket in the cleavage area. Also possibly closing the lower portion of the pocket to allow implant to ride higher - which will help to make nipple appear slightly lower.
