Looking at your photos, a lateral view would be helpful. As far as your lips, a lip lift procedure is the last thing you need as you already have excess dental show and that procedure would only exacerbate it. Adding some volume to the central part of your upper lip would help with this. A rhinoplasty could refine your nasal tip and a conservative alar base reduction may help.

Smoking and drinking during the immediate post op period can affect your healing and swelling. Having said that, this amount will probably not cause disastrous results but it is definitely not a good idea. Also avoid being around second hand smoke for at least the first month. A more important factor would be if you were a regular smoker prior to your procedure then your healing capabilities are significantly diminished. Avoid this behavior for best results.

Pictures would be helpful. Were any implants used in your surgery? Osteotomies(cutting of the nasal bones) to reposition them is a common maneuver in rhinoplasty. Bones then heal by forming a callous which can cause a raised area. This is prevented by proper casting and then with massages after the cast is removed. In general for any facial plastic procedure swelling peaks at about 3 days. With rhinoplasty the tape and cast hold some of the swelling down which then subsides over the first week. When the cast is removed at 5-7 days you can have some rebound swelling though not usually as much as in the first 3 days. 80% of the edema is usually resolved by 6-8 weeks and sometimes sooner. The final 20% can take up to one year.

Rhinoplasty is not a surgery for pain relief. I agree with your need for a spreader graft to support your lateral nasal sidewall but the persistent intensifying pain is a concern. I would suggest a CT scan of your sinuses to determine if sinus disease or obstruction could be the culprit.

I am not a fan of ice after rhinoplasty but I do place an external splint over tape for all cases for about a week. This protects the nose as well as holds external pressure to help prevent swelling. Unclear if you have this on or not. I also use SinEcch (homeopathic Arnica Montana) as this has been shown in studies to help decrease bruising and swelling.
