Hi there!
You may have a residual caudal septal deviation which is why the nostril cartilage (medial crural footplate) is pushed more into the nostril creating a “smaller” nostril. You may have lost some projection or the caudal septal extension graft may have shifted leading to a “short nose.”

Hi there! Nose collapsing following rhinoplasty is typically due to lack of supportive nasal cartilage. Lack of cartilage may be due the request of narrowing the nasal tip, fracturing the nose to narrow at the nasal bones. This can also happen following dorsal hump reduction. There has to be a balance in the external aesthetic and creation of a smaller nose and breathing.

HI Billiebob, yes any patient with more severe medical illness should have surgery at a hospital based setting to ensure that overnight monitoring is available if needed. You would have to ask your plastic surgeon about privileges. Most of us will have privileges at hospital based settings for patients like yourself. Good luck.

Hi there, a rhinoplasty would help you with the longer nose and nasal ptosis with smiling. The length of the nose can be altered surgically to an extent. As far as the dropping nose with smiling a rhinoplasty can also help you out.

Hi there, we would typically send you to a hematologist to evaluate you for blood clotting disorders. The more worry some clots can be very scary and precautions would need to be had prior to rhinoplasty. It could be using a blood thinner during the surgery or any recommendations from the hematologist. typically you could have more bleeding and bruising.

Hi there, yes after a septoplasty the nostrils will not typically change. To change the shape of your nostrils you will need a rhinoplasty. Small changes are more difficult to give.

Yes it’s common to have a small space between the lower cartilages. Typically it would be difficult to damage the ligaments connecting the cartilages unless you had surgery. You might have nasal swelling due to the manipulation however you should have it checked out by a rhinoplasty surgeon if it persists.

Hi there, a septal perforation if it’s small doesn’t usually cause any changes in the nose itself unless it’s near the support systems of the nose. We would need to do an internal exam of your nose to see the perforation. Once the perforation is there it typically doesn’t enlarge unless there is further trauma. So once it’s there the outside of the nose should not change more unless it’s the supportive parts of the nose. Typically it’s more of a saddle nose that can occur.

Hi there. I agree with most answers. The nose structures can weaken over time and changes outside of the nose can make the illusion that the nose has changed. For example if your cheeks were fuller and are no longer full.

An in office exam would give much more detailed information. Typically to determine the excess skin lifting up the eyebrows in clinic is informative to see if the eyebrows are at a good position or too low. In females they should rest above the orbital rim. If you like the results a browlift may be all that is needed. However, if there is still excess skin then a browlift in combination with a blepharoplasty may be needed.

Hello thanks for reaching out on realself. Feminizing the face is about creating harmony between different features. Generally females have a shorter forehead height, so a scalp advancement with small brow lift will give you some benefits. Fuller cheeks on profile view is also very feminine. In terms of the lower part of the face a buccal fat reduction and jaw bone recontouring liposuction would also improve facial femininity.

Hello, sorry to hear about your concerns. I agree with midface (cheek augmentation). Your natural hairline shape is very feminine. Buccal fat reduction would narrow the bottom third of your face as well and jaw contouring.
