Thank you for your question. In looking at your photos, it appears that you may need Lipo and BBL to achieve your desired results. It does not appear that you need a full tummy tuck but perhaps a mini tummy tuck. With the mini tummy tuck your surgeon will be able to improve the loose skin under your belly button and in some cases tighten the muscle leaving you with less of a scar than a full tummy tuck. Just based on your photo, I would recommend that you lose 10 to 15 pounds before surgery to allow your surgeon to remove more tissue and give you an overall better result. It seems that your best option is to address the scar on your belly button. They are not the easiest to fix but typically can be treated with lasers. Raising or lowering the belly button will typically not yield a satisfactory result because the belly button will appear wider or larger and you will have scars above and below the area. Another option would be to remove it all together and get a tattoo to create the look of a belly button.

It is not advisable to get implants over the muscle after having had radiation 9 years prior. Implants should never be put under damaged skin. By putting the implant under the muscle the implant is protected and can provide a collagen sling. The implant over the muscle would make that damaged skin even thinner severely increasing your chances of complications.

Ideally you should do the reconstruction at the time of the mastectomy. If for whatever reason, you do the reconstruction surgery separately,then 3 months or more is typically a good waiting period to put in expanders if you do not require chemo or radiation post surgery as in your case. At this point you should be fairly healed and able to tolerate the reconstructive process.
