Buccal fat pad removal might not be the answer instead an upper cheek and cheekbone area enhancement through a cheek implant, fat micrografting or mini-lift might improve your face geometry and symmetry (as your cheekbone area appears substantially thinner on your right.) Facial scars usually heal better than body scars. If you didn’t depigment, substantially you should do fine with a facelift scar as long as properly designed (wavy, beveled into the hairline, etc…designed.) If properly designed, incision should be quasi invisible and not truly noticeable. Look for examples of my Refresher Lift on my website link.

A lot of the facial aging is genetic. A vertical drop of your cheeks is evident and amplified by a relatively short chin and full neck. Age should not be a concern as the required procedure is a vertical facelift with neck defatting and plication plus chin implant. Done sooner in a younger skin and tissue, the procedure should be more effective and palliate the more permanent aged-induced damages. You are right and your ¾ profile clearly shows a descent and probable deflation of your cheek pads, jowls, and a significant blunting of the neck. You have just spelled out the main facial issues why a facelift procedure is performed. Do not waste your money on fillers, you need lifting of your cheeks and jowls. I do not recommend an aggressive procedure such as the vertical deep plane facelift at this stage. Fillers are no longer the answer. Sagging of the cheeks is not common at your age but sometimes seen after severe weight loss or unusual skin condition. Since your arm fulness appearsand skin looseness  to be located in the upper half of the arm ,a compromise such as  careful liposuction  with an armpit  mini lift (maybe followed with external tightening device if indicated) might very well do the Job !

Looking at your pics   i see more flaring asymmetrical tip cartilages and  very round nostrils .I would probably without the benefit of a  close examination recommend a modification of the tip cartilages and possibly an alarplasty (nostril job) which could  make the nostril look even and more ellyptic than round .Computer imaging would help you visiualize potential results

As most surgeions will agree injectables are implants and in most cases enlarge a given feature.Certainly a reduction rhinoplasty  will reduce  a prominent nose ,I would certainly  consult  someone for the feasibility of a chin augmentation which would certainly improve  your facial balance and relatively  reduce the nose prominence .As i mentioned in other questions computer imaging would help you a  lot  in your decisions !
