At 24 yo it seems unlikely that you have excess skin. However, you may also want to consider lower blepharoplasty with fat repositioning. This will move the fat into the hollow area giving you a smooth transition from the eye to the cheek. Effectively obliterating the dark shadowed area. You should have a complete evaluation by a Board Certified Surgeon in your area.

You are still very early in the recovery process. You need more time for the tissues to heal. We all expect and aim for immediate perfection, however a seroma is a possible risk. I would continue with the ultrasound, however I would not recommend steroid injections.

It sounds like after taking your hump down, your surgeon may have done osteotomies to narrow your nose. Occasionally, that can cause internal nasal valve collapse. A way to improve your breathing could be placement of spreader grafts, which does mean a revision rhinoplasty. I recommend you have a personal evaluation by a Board Certified Otolaryngologist and Facial Plastic Surgeon. This way you can get the look you want but also have someone who understands the function of the nose.

However, you should consider a hylauronic acid filler under your eyes which would help with the hollowness and visually tighten the skin. If you are considering surgery, you should consider at fat repositioning surgery or fat injection to the lower eyelids. Regardless, of what you choose make sure you consult with a board certified surgeon and they should be able to review all your options.

Either way you will have to sign a medical release form. The paperwork is usually handled by the office staff and your original surgeon may never know you requested it.

However, you should discuss it with your original surgeon so he/she can understand what went wrong between you and why you went to another surgeon.
