As noted by my colleagues already, pain patches are more appropriate for patients with chronic pain. There are many options for oral pain medications and treatment regimens that should adequately manage your pain and minimize the risk of side effects. I would agree with many of the answers posted here already. My answer is a general one without the ability to examine you and your particular situation. That said, in my practice, it is very uncommon to use any kind of mesh material in first time or even revision breast augmentation.

As many have already mentioned, hump removal alone can cause middle vault collapse. However, the problem is not always that straightforward. If this is something you have experienced it is best to be evaluated by someone experienced in rhinoplasty and revision rhinoplasty in particular.

I would recommend visiting with a plastic surgeon that has experience going over the muscle with implants. Many of the decisions as to whether one can go over the muscle depend on an examination, and the size and type of implant selected. There are risks to going over as well as under the muscle. There is no risk free surgery and the decision is sometimes based what issues are more acceptable to you as a trade off.

A physician who is there to follow you and take care of you should any problems arise after surgery.
