Crooked noses are difficult to address and the nose will have a tendency to drift back slightly to its preoperative side. Massage can be very helpful in these cases. The swelling on the side of the nose can be one sided and cause the nose to look crooked. At 3 weeks most of the swelling should be gone. It never hurts to see someone else as a second opinion to make sure that the nose is healing well. I would revisit your surgeon and bring up your concerns.

Insurance may cover the surgery if it is from a trauma, but you may also be covered under insurance for the breathing issues. Your nasal bridge may be smaller from the trauma and straightening the nose may include augmentation of the nasal dorsum. If the problem is mostly cosmetic it may not be covered by insurance. Your best bet is to be evaluated by a surgeon that performs rhinoplasty regularly and has good reviews.

A consultation can certainly be done after having the fillers despite the Radiesse injection. Depending on how much Radiesse was injected they may need to wait for the filler to dissolve before performing the surgery. Sometimes the residual filler can be dissolved with Hyaluronidase if it was recently injected.

Crusting is very common after turbinate surgery. It is extremely common to have dryness accompany this. Your surgeon can remove these crusts in the office, although not every surgeon does this as it can increase bleeding. I often recommend for my patients to start a salt water nasal rinse after this type of surgery to cut down on the crusting and dryness.

Alar base reduction is a separate procedure and can be done with or without tip work. However if you are concerned about more than just the alar base and have concerns about the tip then you should wait until you are able to do everything at once. Having the columella protrude on the left side could still be swelling. The firmness should continue to improve. It can improve up to a year and beyond. Having said that, the majority of the swelling should be gone by now. If you are concerned about the firmness but pleased with the overall shape and form of the nose, then I would wait. If you feel that the nose still looks swollen, I would have it evaluated by your physician.
