Scar revision is always an option. Without looking at your nose, I can't tell exactly what would be the best option but dermabrasion and simply excising the scar and starting over are always options. I would suggest visiting with a facial plastic surgeon to discuss your options.

There are many reasons for nasal airway problems. Just one week after surgery, it may be normal to have decreased airflow. There could still be scabs in the nose, the surgeon may have used absorbable packing to stop bleeding, or there may be swelling. On the other hand, there may be bleeding that needs to be controlled or a remaining deviation that needs to be surgically addressed. I would certainly talk with your surgeon about your concerns and have them reexamine your nose.

I would most certainly suggest speaking with your doctor about your concerns. Implants can move over time and it may be pressing on the mental nerve, the nerve that provides sensation to the chin and lip. This could explain your problem with numbness and pain. If this is the case, the implant needs to be repositioned or removed and then replaced.

I would certainly agree with your doctor to wait at least 3 months, and possibly longer, for all of the swelling to resolve before jumping into another procedure. There should be no long term problems with waiting a few extra months before jumping to removal if you do not like the implant.

In general, skin discolorations can be treated with either lightening creams or laser treatments, even in African American skin. If there is firmness of the lobule due to scar tissue, steroid injections may be helpful to improve the appearance of the area.

Unfortunately your ear position is going to be determined largely by the location of your ear canal which can not be easily moved. The ear can be reshaped to some degree but usually not 'moved' more than a few millimeters. I would still suggest speaking with a facial plastic surgeon with experience not only in otoplasty but microtia repair as you may be able to reshape part of the ear to create the illusion of moving your ear.

As Dr. Kontis noted, Artefill is a combination product. For full disclosure, I don't use Artefill because I feel I can get the same permanent results with fat transfer to the tear trough as others may get with Artefill. I have seen several patients in whom the Artefill has migrated or become infected and needed removal. I would agree that every procedure, including fat injections, have complications and some of these risks like lumps and bumps are often injector dependent.

While I don't understand exactly what is meant by 'not working' it is certainly reasonable that additional swelling can exist 3 weeks after surgery. I would suggest close follow up with your physician and continued patience.

Implant migration can occur with any type of artificial implant. At just two weeks, I would certainly agree with your physician that conservative measures are best to try to realign the implant. In some cases, uneven looks and feel can simply be due to swelling which will resolve over several weeks to months. If this does not work, revision surgery can always be undertaken but I would only use surgery as a last result.
