Time Frame for blepharoplasty and age safety
All about blepharoplasty
Feb 3, 2017
Blepharoplasty is a safe surgery that lasts many years when properly performed. The eyelids definitely do not “droop” again since the skin that is removed is gone forever. After upper blepharoplasty done after the age of 50, it is rare that another upper blepharoplasty will be needed in your lifetime. However, the eyebrows do descend with time so some patients will choose to get a browlift when they droop sufficiently to warrant it. Lower blepharoplasty also lasts many years, but since fat i…

Revision rhinoplasty with asymmetry and retraction
Feb 3, 2017
Hi. I think you intended to include photos but I don’t think you were successful in doing that.Nonetheless, revision rhinoplasty can address many if not all of your concerns. A personal consultation is always best in order to get the best idea about what surgery can and cannot do.
Revision Rhinoplasty Diced Cartilage wrapped in Fascia
Diced Cartilage Wrapped in Temporalis Fascia in Revision Rhinoplasty
Sep 2, 2016
Diced Cartilage Wrapped in Temporalis Fascia (DCTF) is a superb way to increase bridge height. The graft feels natural once healed, and far superior to rigid rib, bone or other cartilage grafts. The survival is excellent in the long term. As in any graft, there are possible complications, but the DCTF is less likely to have typical graft complications (edges showing, shifting, loss of correction) than other dorsal grafts in revision rhinoplasty.
My right eye is sunken in. Is it possible to get it fixed?
Eye asymmetry
Sep 2, 2016
Asymmetry is normal between the two sides of one’s face. This is usually just due to natural bony asymmetry. If the change in your eyes has occurred suddenly, then best to consult with an plastic, oculoplastic or facial plastic surgeon. A CT scan may be required. However, if your concern is only cosmetic then it is unlikely that anything would be done to significantly change your asymmetry. Natural asymmetry is just that; best to leave it alone.
Are wrinkles and hollowness a common complication in transconjunctival blepharoplasty?
Wrinkling after transconjunctival blepharoplasty
Aug 16, 2016
Transconjunctival blepharoplasty is more likely to result in wrinkling of the skin than is transcutaneous blepharoplasty. As fat pushes the skin outward over time, the skin will stretch a bit. Add to this the loss of collagen and elastin due to aging and you are set up for more lines and wrinkles after any blepharoplasty. In my experience, skin pinches, which remove just a little skin from the upper edge of the lower eyelid, do nothing to lessen the lines/ wrinkles on the rest of the lid. I…
Can a Revision Rhinoplasty help me?
Revising an overdone, over-narrowed rhinoplasty
Aug 10, 2016
Your nose is overdone. The bridge was over-narrowed, the cartilage side walls (upper lateral cartilages) have been weakened at their dorsal attachments, your tip projection is increased and over-rotated, and your nostrils have widened and retracted. The good news is that revision rhinoplasty can correct most, if not all, these problems. The caveat is: you need to choose your revision surgeon wisely. Consult with a board-certified facial plastic surgeon or plastic surgeon with a lot of…
Follow-up Question: Why does my nose do this post primary rhinoplasty and can this be fixed wi
Droopy tip after rhinoplasty
Aug 4, 2016
Your photos show a heavy, droopy tip with excessive fullness above the tip. This can occur after rhinoplasty when the support your surgeon used could not stabilize the nose sufficiently. Revision rhinoplasty would require significant strengthening and defining of your tip with cartilage grafts, with possible excision of soft tissue scar. Consider seeing a board-certified rhinoplasty surgeon with experience in performing challenging revisions.
How do I get rid of the sunken eye looking area under my eyes?
Sunken Eyes: Fillers vs Surgery
Aug 4, 2016
If you have sunken eyes, then filler can provide good improvement that lasts about one year. Surgery also works and is more permanent, but at a young age usually filler is the recommended start point. Filler can also help enhance the lips. Your photos do not show your droopy eyelid well. For an opinion about your eyelids see a board-certified facial plastic surgeon, plastic surgeon or oculoplastic surgeon, since this is often a surgical problem.
Will a second rhinoplasty narrow my bridge from my front angle?
Revision rhinoplasty for bridge narrowing
Aug 4, 2016
Your nose appears wide primarily in the middle and lower thirds. In these areas, width is due to the cartilages of the nose. In addition, you seem to have cephalically malpositioned tip cartilages. In this case, the width is often difficult to correct. However, experts in rhinoplasty can correct your width without creating new problems with nasal obstruction. See a board-certified surgeon who is experienced with revision rhinoplasty, but you can probably achieve additional improvement with…
Am I a candidate for under eye filler or a lower lid blepharoplasty?
Lower eyelid bags plus tear troughs
Jul 21, 2016
Your photo suggests you are a candidate for either filler or blepharoplasty. However, if the tear trough is deep, then you may also benefit from a trans-blepharoplasty midface lift. With a much of a bulge of fat as you have, you may also benefit from lasering or chemical peels during surgery to help prevent new lines from appearing after you heal. You should definitely consult with a facial plastic surgeon or oculoplastic surgeon to identify what your best choices are.
One of my eyes is dropping. What is causing this/can it be fixed?
Asymmetric tear troughs at a young age
Jul 21, 2016
You are correct, you have asymmetric tear troughs. This is most likely due to asymmetry in your underlying bony structure around this area, plus how the muscles and fat in the area drape onto the bone. Sometimes these asymmetries are exacerbated by weight loss. The best first step is to see a facial plastic surgeon or oculoplastic surgeon for an examination. Sometimes fillers can help correct this problem; sometimes surgery can help.
What corrections would you recommend for post septorhinoplasty contour irregularities?
Bony-cartilagenous irregularities after hump reduction rhinoplasty
Jun 29, 2016
Irregularities at the transition point on the dorsum between where the bone ends and the cartilage begins are common. This is because the skin is thin here, so any small irregularity shows. You cannot decide if the protrusions are bone or cartilage because they may be either, or both! Fortunately, in your case they are very minimal. 10 weeks after surgery is early to consider treatment, but if they continue to bother you in a few months then your surgeon might suggest a small rasping…
Is my only option at this point a revision? Can a good result be achieved or should I leave we
Patience is Key Before Committing to a Revision Rhinoplasty
Jun 29, 2016
Four months after primary rhinoplasty, you are still early in your recovery. In thick-skinned patients, revision rhinoplasty should not be considered until the skin has softened completely. This is rarely earlier than one year after surgery. If your surgeon feels that Kenalog injections may be helpful, then do not ignore his advice. Kenalog at this stage can be very helpful in reducing scar so that revision surgery is easier, with a higher chance of success. Discuss your concerns with your…
What can a revision rhinoplasty do for me?
Reversing Rhinoplasty
Jun 23, 2016
Once you’ve had a rhinoplasty, your old nose is gone. Even if no cartilage had been removed and your cartilages were merely bent with sutures, after 3-6 months scar tissue will have bound your cartilages into their new shape. The tip can be made fuller by adding cartilage for volume, but it will not be your old nose. Revision rhinoplasty is more difficult than primary rhinoplasty and can lead to unexpected results, so be sure about it before having another surgery.
